Job summary

We are seeking an experienced Band 6 Health Screening and Community Nurse to join our Community Learning Disability Team on a 12-month fixed-term contract for maternity cover. Full-time (37.5 hrs) with a blended focus:

Three days per week delivering health screening services, focusing on supporting service users, families, and care providers to engage with the five national screening programs across settings. Your role will include advocating for reasonable adjustments, using enhanced communication skills, and creating pathways to ensure equitable access to screening. You will also deliver specialist educational support and resources to mainstream services, promoting inclusion and better health outcomes.

Two days per week providing community nursing care as part of a multidisciplinary team (MDT). This senior clinical role is accountable for clinical effectiveness and service development for individuals with complex needs. You will deliver holistic care, covering both physical and mental health, through case management, person-centred planning, and coordination of support. You will liaise with agencies and providers to ensure adjustments and continuity of care.

Main duties of the job

Health Screening Role (3 days per week) This post focuses on improving uptake of the five national screening programmes for individuals with learning disabilities and supporting timely access to checks that promote early identification and treatment. You will work with GP surgeries, hospitals, screening teams, and third-sector organisations to reduce health inequalities. Working alongside National Screening Leads, Learning Disability Services, Care Coordinators, and clinical leads, you will review GP LD registers, identify cohorts for proactive contact, and use innovative approaches to raise awareness of screening programmes and their role in improving health outcomes.

Community Nursing Role (2 days per week) As part of the MDT, you will assess, plan, and manage risks for individuals care needs, including physical and mental health and behaviours that challenge services. This senior clinical role includes case management and care coordination for those fully health-funded, liaising with commissioners to ensure needs are met. You will advocate for individuals, ensuring reasonable adjustments by mainstream services, and work in partnership with families and providers while maintaining safeguarding and best-interest decision-making in line with legal frameworks.

You will review GP LD registers, standardise inclusion criteria & identify cohorts of individuals for targeted contact and proactive intervention.

Working closely with the Learning Disability population using an innovative and proactive approach to raise awareness of the 5 National Screening Programs & the vital role that they can play in reducing health inequalities.

This is an autonomous role which involves working with in a consistently cohesive & collaborative way creating new pathways & processes that will ensure that there is a quantifiable improvement in access to screening & follow up care for those with a learning disability.

Working flexibly across services for people with learning disabilities & at the interface between primary & secondary care.

Acting in a liaison role as required & as part of the MDT ensuring that health screening checks are completed for all eligible individuals with a learning disability where able.

MAIN DUTIES/RESPONSIBILITIES

Assess, plan, implement & evaluate treatment safely & effectively, in partnership with medical team & nurses, service user & carers, using evidence-based practice.

To ensure comprehensive risk assessment of patients & risk management plans are in place & clearly documented in accordance with the Trusts Policy.
