An exciting opportunity has arisen to come and join the Kingsbridge Community Therapy Team as a Band 5 Physiotherapist.

We are based at South Hams Hospital on the outskirts of the beautiful town of Kingsbridge. We are fortunate to be based within the community hospital, working alongside, District Nursing and in-patients and out-patients teams, this enables us to provide a truly holistic and collaborative service to our patients, mostly from the local surrounding area, which is a joy to work within.

The Kingsbridge team has pleasant office facilities, free car parking on site, pool cars and provide an excellent community therapy service to the local, and rural areas surrounding Kingsbridge. We are a small, but friendly and supportive team, and it is an enjoyable place to work, offering either an experienced or newly qualified Band 5 Physiotherapist, the opportunity to join a knowledgeable, hard-working and great team.

The post holder will need to travel and attend to service users based across Plymouth and other geographical areas. Full and valid driving licence and access to a car for work purposes is required. Main duties of the job

We have an exciting opportunity for a Band 5 physiotherapy, to work within our Therapy team, alongside OTs and Therapy support workers.

You will need to be an excellent communicator, competent in a wide range of clinical areas especially the frail elderly with multiple pathology & end of life care. You will need to demonstrate flexible problem-solving skills, ability to work alone or with other team colleagues e.g. Occupational Therapists, Speech &Language Therapists, support workers & Health &Social Care Co-ordinators.

The uniqueness of working in Kingsbridge, allows for you to gain & develop skills in urgent care, hospital discharge, Community Hospital, Pulmonary Rehab, Falls & a variety of therapy rehab approaches; so variety is in abundance here.

We are a service with a strong emphasis on evidence-based practice, in-service training, individual performance review &personal development planning. We provide excellent support through regular line management meetings, post-graduate education & practice supervision.

The team benefits from close working relationships with the wider MDT, such as Acute Response Practitioners, Health & Social Care, District Nurses, Long Term Conditions Matron & GPs.

All staff are expected to able and willing to work across a 7 day service.

Clinical

To problem solve and liaise as soon as possible to the most appropriate clinician for problems that may arise.

To review with Band 6 Physiotherapist any clinical issues or concerns.

To maintain accurate records, which are confidential, contemporaneous, legible and all care given to be documented.
