A full time B5 Physiotherapy position within a fabulous and innovative team has become available and they don't come along often. Have you ever thought about working in the Community? Are you passionate about supporting people in their own homes to live as independently as possible?

If so then we could be the team for you - an exciting opportunity has arisen to join a forward thinking Multi- Disciplinary Team where patient centred care is paramount. We deliver rehabilitation in our Locality in a number of ways:
Intermediate Care and Home First - urgent intervention for admission avoidance and supporting hospital discharges delivered by Occupational Therapy, Physiotherapy and Therapy Support Workers. Close working relationships with the Health & Social Care Hub, Rapid Response Team and the Acute Hospital.
Rehabilitation within the patients own home assessing and treating a variety of conditions e.g. neurological, long term conditions, orthopaedic, frail elderly, multi-pathology, falls, respiratory.

Main duties of the job
The post holder will be well supported by senior staff as well as the wider multi-disciplinary team, to develop their clinical expertise in Rehabilitation in the Community with rotations through the various aspects of Community Therapy listed above. There are excellent opportunities for Continued Professional Development, through shared learning, shared skills and formal educational opportunities. Caseload supervision for junior staff and participation in student education will be part of this role.

Flexible working across all service areas will be an expectation of this post. We also work closely with University Hospital Plymouth to enable early hospital discharges where appropriate.
Located on the edge of Dartmoor National Park, we are extremely lucky to work in an area of outstanding natural beauty. Many of our patients live in rural locations, so being able to drive and having access to a vehicle will be essential for this post.
