22.5 hours per week.
The Complex Needs Team is a mental health team that
support people with co-occurring mental health & substance use issues. The
team provide assessment of needs, development, implementation and evaluation of
care for patients from a specified group of patients who have a diagnosis of a
severe and enduring mental health condition with co-occurring substance use
issues. In addition to complex needs, high risk behaviours' and difficulties
with engaging with services.
The service provides a Plymouth city wide service. The
Mental Health Practitioner role provide a holistic assessment, evidenced based
interventions and assertive support for people with co-occurring mental health
& substance use issues.
The post holder will work alongside the other
services within the Plymouth Alliance that support clients that experience
multi disadvantage and live complex lives within the community. The aim of the
work the Complex Needs Team undertake is to promote recovery from dependent
substance use and improve the health and wellbeing outcomes for individuals,
linking them in with community activities and wider support networks.
The role
will work across the Plymouth Alliance in a variety of settings, in homeless
provisions, community setting and alongside VCSE partners. There is an
expectation that staff may be required to work in other parts of the
organisation/complex lives system on request according to service need and as
part of personal development.
Main duties of the job

To provide of line management
& caseload supervision to Band 5 & below.
To contribute to professional
development, appraisal, performance review and recruitment.
The post holder will act as a
Care-Coordinator for a defined group of patients and organise care in
accordance with the Personalised Care Framework.
To contribute to professional
development, appraisal, performance review and recruitment.
The post holder will ensure
that their clinical practice is based on evidence and good practice and in
accordance with any NICE guidance.
The post holder will be
individually accountable for their practice in accordance with the NMC, HPC,
Social Work England professional code of conduct.
To promote positive risk to
allow clients independence and autonomy of choice.
To liaise with other
community resources and facilitate use of mainstream resources.
To maintain a caseload of
clients with a significant degree of mental health complexity.
To participate in the shift
lead and provide appropriate supervision.
