Are you a dedicated and compassionate Mental Health Nurse with a passion for working with individuals with learning disabilities?

We are pleased to offer a Band 5 to Band 6 Development Opportunity for a motivated nurse to join our specialist service supporting people with learning disabilities and complex mental health needs.

This development post is designed to support your transition from Band 5 to Band 6 through a structured programme of professional development, clinical supervision, and mentorship. You will work as part of a skilled multidisciplinary team, delivering person-centred, holistic care and promoting recovery, independence, and improved quality of life for individuals with a range of learning disabilities and co-occurring mental health conditions.

The role will provide you with opportunities to enhance your skills in care coordination, positive behavioural support, risk assessment and management, and collaborative working with families and other agencies. You will be supported to build the leadership and clinical competencies required for a Band 6 role, with regular progress reviews and tailored learning objectives.

This is an excellent opportunity for a nurse who is committed to making a difference and developing their career in a supportive, specialist environment. On successful completion of the development programme and achievement of required competencies, you will progress to a substantive Band 6 position.

Main duties of the job

The Community Nurse is responsible for assessing, planning, coordinating and reviewing a caseload of adults with learning disabilities. They may have either or both physical and mental health care needs. This will include relationships with Primary and Secondary health care, as well as private and voluntary care agencies.

To be responsible for providing clinical support whilst promoting the effective and efficient delivery of high quality outcome focused service in a person centered approach.

This is a generic community care role providing support and cover across Plymouth city localities.

There is responsibility for participating in reviewing a persons eligibility for Continuing Healthcare funding and is responsible for making consistent and well-informed professional decisions about the determination of CHC funding, ensuring that the national framework is applied and they adhere to all local policies and procedures.
