37.5 hours per week.
The CNRT deliver patient-centred MDT neuro-rehabilitation to patients with neurological diagnosis across Plymouth, South Hams and West Devon. We are a passionate MDT consistent of physiotherapists, occupational therapists, specialist nurses, speech and language therapists, neuro-rehab practitioners, neuro-rehab assistants, neuropsychology, neuro-rehab medicine and administrative team. We are driven to help patients maintain their independence and quality of life by delivery patient goal-based rehabilitation programmes.
You will contribute in the delivery of rehabilitation under guidance from registered professionals to support patients achievement. This will be achieved through implementing a variety of therapeutic interventions and practise of activity of daily living depending on patient's needs.
You will provide support and supervision in all aspects of rehabilitation to patients. The delivery of rehab is delivered in a variety of settings including CNRT outpatients and gym at Mt. Gould Hospital or within community setting or patients home.(this may include, 1-1, group sessions or joint working with colleagues).
You will work with all patients referred into the service with a variety of neurological conditions such as Stroke, Multiple sclerosis, Parkinson's, Traumatic Brain Injury and Spinal Cord Injuries. You will support running of the department including admin responsibilities and infection control measures.
Main duties of the job
Work as a neuro rehab assistant in outpatient and community-based rehabilitation assistant and undertake an active role in planning, co-ordination, delivery, and evaluation patients' neuro rehabilitation.
Follow care plans from registered staff for occupational therapy, physiotherapy, speech and language therapy and nursing interventions to support the rehabilitation of patients.
To assist in the provision of a flexible based therapeutic activity programme as part of the service provision to patients using the service.
Liaise with members of the MDT and wider services both internally and externally where necessary.
Facilitate individual activities for specified patients working within risk management guidelines, which includes working alone for periods of time under the guidance of a registered professional.
Contribute to audits to support evidence-based practice and research projects to enhance clinical practice.
Have awareness of and follow the Health and Safety at work act, local and national guidelines, policies and procedures.
Participate in all mandatory or role specific training.
Assist in collection and maintenance of statistical data this could be in the form of therapeutic outcome measures, stroke data and operational requirements (e.g. audit).

Maintain accurate and complete patient records in accordance with organisational guidelines.

Form a rapport and professional relationship with patients, maintaining clear boundaries for effective working practices.

Work with groups and individual patients using a range of treatment and interpersonal skills, demonstrating empathy and good observational skills to assist an individual to achieve their potential/goals.
Record contacts, interventions and observations on the electronic record systems.
Contribute to the assessment, planning, implementing, and evaluation of care to patients.
