Job summary

  • We have a vacancy available for a 37.5 hour post.
  • The Community Crisis Response Team is based at Mount Gould Hospital and is a team of approximately 60 staff members of varying professions
  • The vacancy is a permanent contract for a Community Support Worker who support clients in the community and gone into crisis and to help avoid hospital admission
  • The shifts are variable between 8.00am and 8.00pm
  • To work as a generic support worker undertaking specific tasks to support clients receiving intermediate care. The post holder will be required to work alone with clients in various settings in the community without direct supervision

Main duties of the job

  • To take a holistic approach to client management, carry out tasks and treatment plans as directed by senior CCRT staff. To feedback to the appropriate CCRT worker and maintain accurate clinical records
  • To support clients with achieving intermediate care goals within 6 week intermediate care period
  • To undertake client interventions and reviews by CCRT workers, regularly meeting the relevant professionals to discuss progress and reporting back any problems occur
  • To carry out multi-disciplinary treatment plans on behalf of the team
  • To seek advice and supervision from the CCRT senior worker within the team as necessary to ensure appropriate patient management and care
  • To raise safeguarding alerts in line with LWSW procedure
  • To support and encourage patients in active approach to regaining and maintaining maximal functional independence
  • To work closely with other health and social care colleagues and service providers to ensure patients identified intermediate care goals are met
  • To actively participate in personal development training needs

MAIN DUTIES/RESPONSIBILITIES

To be responsible for contributing to the planning/monitoring/evaluating of patient centred care. Reporting back to the registered nurse & escalating concerns as required.

Following a Registered Nurse or Assistant Practitioners assessment of need the post holder will then implement a planned programme of care, with the support of a Registered Nurse.

To act as a role model for all staff/students demonstrating clinical skills & high standards of practice/professional conduct.

To take responsibility for your own personal/professional development, maintain competence, knowledge &skills commensurate with the role.

To participate in Reflective Practice, Practice Supervision & Appraisals & line management supervision as per policy.

At each visit staff risk assess the need to move light furniture in patients home to allow nursing procedures to be carried out often within a confined space.
