37.5 hours per week.

A very exciting opportunity has arisen to join the Integrated Hospital Discharge Team at Derriford hospital. This most is covering a six month secondment within the team.

We work in a fast paced environment to ensure patients are discharge out of hospital safely and in a timely manner.

We are a Integrated Transfer of Care Hub (ITOCH).The Transfer of Care Hub is the local health and social care system-level coordinating centre (fully or partially co-located with acute settings where suitable) linking all relevant services across sectors to aid discharge and recovery and admission avoidance.

The team compromises of nurses, social workers and, discharge case managers who coordinate and facilitate patient complex discharge from hospital.

You will be responsible for the day to day running of the team and to coordinate complex discharges.

Main duties of the job

The post holder will be responsible for management and

Provide business support to the Integrated Hospital Discharge Team. Manage the coordination of the complex case load knowledge and understanding of caseload pressures according to ward areas.

Advise and provide support to Discharge Case Managers to ensure they adhere to process and discharge standards according to Care Act and National Discharge Guidance

Coordinate all Complex Discharge referrals. Linking previous support packages to support MDTs in discharge planning.

Manage the day to day recording and verification of delayed transfers of care project.

Manage failed discharges and find appropriate solutions.

Manage the day to day recording, verification and reporting of Delayed Transfers of Care.

Ensuring the delivery of seamless high, quality service to patients and clinical teams.

To provide managerial and administrative support to the integrated hospital discharge team managers. To include sickness, appraisals and ensuring appropriate levels of staffing.

To take responsibility of investigating and answering complaints.

To support the managers with staff training and development.

To support the IHDT to maintain patient flow from the acute hospital to their pathway destination, to escalate any barriers to the appropriate organisation.

To triage and coordinate referrals and facilitate the correct discharge pathways.
