We are
looking for a dynamic and person-centered Community Care Worker to join our
award-winning Urgent Care team. This is an exciting time to join the Community
Crisis Response Team as investment allows expansion of roles in admission
avoidance and the development of exciting new initiatives. All roles are valued
members of this integrated team and you will gain great satisfaction in using
your skills, in collaboration with trusted health and social care colleagues,
to work with patients during their health crisis.
The team
includes Physiotherapists, Occupational Therapists, General Nurses, Mental
Health Nurses, Social Workers, Community Care Workers, Paramedics, Support Workers
and coordinators. In addition, there are opportunities for working with Acute
Response Practitioners within the team to expand your understanding of physical
assessment and clinical reasoning of frail elderly patients in primary care.
CCRT
promotes a culture of learning and improvement, and as part of our team you
will have the opportunity to gain desirable interdisciplinary skills whilst
maintaining your social care identity. Staff are supported with a regular
training programme to meet team competencies, as well as encouragement to
attend relevant professional courses.

Main duties of the job
The post holder is required to discharge
statutory social care functions on behalf of the organisation. They are
required to undertake non-complex and proportionate assessments that are
evidence-based. The term non-complex is defined as - predictable situations
often where the person is motivated to engage and respond to information,
guidance, and advice. The person may present a low level of risk to self or
others and the risk can be managed safely. These situations often require
short-term interventions, and the decisions require some analysis of different
views and factors impacting on the persons life.
Examples could include mental capacity
assessments and health and social care assessments of people who have a variety
of needs. The post holder is also required to arrange and review the delivery
of social care support. There may be situations where the work is complex and on
these occasions the post holder will need to seek support from social work
colleagues or their line manager.
All Livewell Southwest staff are expected to able and willing to work across a 7 day service.
