An exciting opportunity has arisen for an Integrated Operational Lead within our vibrant Community Integrated Service. This role has arisen from our ground-breaking Shaping Our Futures Programme, which aims to foster seamless collaboration between health and social care teams in Adult Frailty and Specialist Services.
Our Community Integrated Teams play a crucial role in enabling individuals to thrive in their own homes or care facilities, promoting independence for as long as possible and preventing unnecessary hospitalizations and long-term care placements.
You will be right at the heart of things, working together with Directorate Managers, accomplished B7 Nursing, Adult Social Care, and Therapy Leads, and our outstanding Frailty Team, to achieve our objectives and ensure excellent outcomes for our service users. As a key player in our integration journey, your wealth of experience in health and social care, coupled with your exceptional leadership skills, will shine through. Your talent for collaborative working will be pivotal as we drive initiatives to enhance our systems and structures.
This role calls for adaptability, creativity and willingness to undertake a range of tasks and roles as the functions of the post and service develops. You will be able to use your initiative and work autonomously as well as working as part of a team. This challenge promises not just growth, but also the opportunity to shape the future of our service and your role within it!
Main duties of the job
It is essential that interested candidates are able to work flexibly and creatively in a changing environment, are able to use their initiative and work autonomously as well as working as part of a team, and are willing to undertake a range of tasks and roles as the functions of the post and service develops. Whilst this will be challenging, it is hoped that it will also enable scope for the successful candidate to help shape exiting developments within the service and the role.
