Job summary

37.5 hours per week. Livewell SW provides integrated health and social care services for people across Plymouth, South Hams and West Devon, as well as some specialist services for people living in other parts of Devon and Cornwall

The team have recently gained financial investment from our Integrated Care Board to realign and reshape the support offered to patients in Plymouth, South Hams, West Devon and East Cornwall to meet the National service model for an Integrated Community Stroke Service (ICSS)

The community provision for Early Supported Discharge (ESD) and Neuro Rehabilitation (NRT) is at the start of a transformation journey that will see a focus on improving the care of patients

The ESD service provides multidisciplinary specialist stroke rehabilitation for patients post hospital discharge

The NRT service provides community and outpatient specialist neuro rehabilitation

The team are currently hybrid workers with opportunity to work from home and office when not treating patients in the community

The team covers a wide geographical area including Plymouth, Devon and East Cornwall

We are seeking a highly skilled, enthusiastic and motivated band 7 Occupational Therapist to join the team and work across the ESD and NRT based at Mount Gould Hospital

This is an exciting opportunity to participate in redesigning the pathway and developing the service

Excellent communication, computer literacy and accurate documentation skills alongside specialist clinical skills are essential.

Main duties of the job

To act as a lead therapist responsible for providing a comprehensive specialist occupational therapy service to patients requiring community and outpatient neuro/stroke rehabilitation

To deliver the highly specialist clinical needs required for the post, through established experience/specialist postgraduate training

To act as a source of expertise on neurological conditions (including brain injury, stroke, degenerative conditions and spinal conditions) of adults in the community; providing advice to patients, carers, physiotherapy colleagues, other health care professions, consultants, GPs and social services

To maintain high professional standards and continuously promote/develop quality improvement appropriate to the needs of the service

Support self and service development through active participation in continuing professional development and participation in any audit or research being undertaken by the service

Make recommendations and lead on any changes in practice within the team and contribute to the development of service protocols

Supervise, educate, and assess the performance of junior staff

Carry out highly specialist, comprehensive assessments using investigative and analytical skills, of patients with a complex presentation for adults with neurological rehabilitation needs in the specialist neuro and stroke outpatient/community setting

Use advanced clinical reasoning, analysis, assessment and re-assessment skills, using a patient-centred goal setting approach; to develop and provide individual occupational therapy treatment programmes. These may include detailed assessment of upper limb function, functional electrical stimulation, highly specialist standardised and functional cognitive assessments, complex upper limb splinting, complex functional assessments, assessment for specialised equipment and patient /carer education

Evaluate patient progress, reassess and alter treatment plans as required

Provide highly specialist advice via written or verbal instruction to patients, carers and relatives to promote understanding of the aims of rehabilitation to ensure a consistent approach to patient care

Use an extensive range of verbal and non-verbal communication tools to communicate effectively with patients/carers; to motivate and gain co-operation to progress rehabilitation and treatment programmes

Advise on and prescribe both general and specialist rehabilitation aids, equipment and recommend low-level adaptations, ensuring all equipment is maintained and used effectively and efficiently with risk assessments and appropriate training completed

Demonstrate highly developed clinical reasoning skills to undertake risk assessments within the outpatient/community settings, identifying and making recommendations as required

Assess the risk and vulnerability of patients/families in relation to timing and manner of treatment, and amount of treatment provided i.e. where inappropriate intervention could have a negative effect

Use highly specialist occupational therapy knowledge and skills to manage own complex caseload, as an autonomous practitioner and to organise this effectively and efficiently with regard to clinical priorities and use of time

Using hoists and other heavy gym equipment daily, often in a confined space in the patients home on difficult floor surfaces

Risk assessment of home environments

The job may involve exposure and contact to unpleasant working condition such as secondary smoking, bodily fluids, including sputum, vomit, blood, urine and faeces, soiled linen, fleas, lice, scabies poor hygiene standards and communicable diseases

Possibility of exposure to verbal and physical aggression and dangerous pets

Exposure to heat and cold in patients homes and the outside environments such as standing on a patients doorstep awaiting entry

Food handling

Mobile phones may not work in areas where there is poor network reception
