Main duties of the job

We are seeking a registered mental health nurse who possesses recent clinical experience in a community or relevant mental health inpatient setting.

SORT are working towards an enhanced, recovery and transitional offer within the team. The staff team will be expected to rotate through the 3 levels of the service and dependant on their interests, skill set and needs of the service. This will provide the candidate a variety of experience but remaining part of a larger team with the main focus on mental health community recovery.

SORT are no longer working under the CPA framework, we are one of the first teams to adopt PCF with Livewell, with an emphasis on key working depending on professional registration and the needs of the service users.

Job description

Job responsibilities

Responsible to the Manager of the Specialist outreach recovery team for the assessment of needs, development, implementation and evaluation of care for patients from a specified group of patients. The practitioner is responsible for providing key worker intervention to a defined group of people with a complex mental health problems in a community setting. To provide caseload management and support to a defined caseload, and also to provide teaching and supervision to students and others undertaking educational programmes.

  • Provision of case load supervision for Band 5 mental health practitioners and line management.
  • Contribute to professional development, appraisal, performance review and recruitment.
  • Responsible for the management of SORT caseloads. Carry out reviews of caseloads and support the process of recovery.
  • Promote positive risk to allow clients independence and autonomy of choice.

  • Liaise with other community resources and facilitate use of mainstream resources.
  • Maintain a caseload of clients with a significant degree of mental health complexity who are difficult to engage in services.
  • Participate in the shift lead and provide appropriate supervision.
  • Practice within the legal and ethical framework set out by mental health related legislation to ensure that service users interests and well-being are at the fore.
  • Liaise, advise, develop and maintain good relationships with service users, carers, relatives and fellow health, social care professionals who are involved in the delivery of services.
  • Participate in all activities aimed at keeping professional skills updated and contribute to dissemination of that knowledge and experience to all staff and students within your area of responsibility.
  • Act upon referrals from secondary care, by providing assessment, implementation and development of programmes of care to individuals, groups and families.
  • Implement and participate in the formulation of all policy and procedures within the area of responsibility in consultation with relevant others.
  • Plan, with teaching establishments, a realistic learning experience for students and for postgraduate students and be responsible for the quality of the programme.

  • Monitor standards of practice within the team, ensuring areas of deficit are addressed through audit.
  • Co-ordinate the collation of evidence within the team in support of standards for better health improving quality.
