Job summary

An exciting opportunity has arisen within the Kingsbridge Community nurse Team for a band 5 Community Registered Nurse

Up to 37.5hours per week.

The role requires positivity and excellent communication, leadership, and caseload management abilities. You will need to demonstrate integrity and initiative, sound clinical knowledge with evidenced based practice and the ability to manage a team, approaching your work in a coordinated way.

We work integrated with primary care, adult social care, therapists, urgent & intermediate care services alongside our specialist services informing our ageing well MDTs which facilitates shaping our future to deliver great quality care for housebound individuals.

Through expert advice and clinical support and treatment, promoting the optimum quality of life. Whilst providing an effective and efficient service delivery, utilising health promotion, wound, medication, and continence management.

Main duties of the job

The post holder works as a member of the District Nursing &Primary Health Care Team during their working day.

The post holder is supported by the Caseload Holder &is responsible for providing skilled nursing care &information to patients &carers.

The aim of which is maintaining patients in their own homes & enabling them to enjoy maximum independence &optimum quality of life.

The post holder will have a clinical role within the District Nursing Team; they will have responsibility for admitting &discharging patients to and from the caseload(s).

The post holder is responsible for assessing, triaging, planning, evaluating &implementing care which may be complex, sensitive &unpredictable with the support of a Caseload Holder.

The post holder is responsible for coordinating the caseload in the absence of the Caseload Holder on a temporary basis. This will be reviewed after a three-month period by the line manager or a Manager.

The post holder will work alongside Health & Social Care Services within statutory and voluntary sectors providing appropriate nursing skills to ensure care packages are responsive to individual needs of patients within their own homes &other care settings.

To signpost to appropriate services, statutory & voluntary bodies.

To undertake &to develop all nursing skills &competencies, updating as required for the role to enhance the skill mix of the nursing team, ensuring that the ever-changing health needs of patients are met.
