Communication
and Relationship skills:
Detailed confident
verbal information sharing, within ASR, third sector, government
agencies and south west collaborative or the wider Multi Disciplinary Team,
which includes Primary Care.
Expert ability in receiving information,
analysing and formulating professional judgements.
Attend and represent the Teams view in
meetings e.g. Child protection, Adult Protection, Mental Health Review
Tribunals, CPA Reviews.
Elicit information from the patient and
work with them to advocate on their behalf.
Ability to communicate clearly and
effectively over the telephone within the boundaries of confidentiality.
Responsible for communication with staff
within the Team ensuring that interpersonal relationships are managed so that
Team dynamics have a positive effect on patient care.
Maintain accurate, timely paper and
electronic records. Ensure they are kept
safely in accordance with Livewell Southwest policies, including the Data
Protection Act (1998) and the Livewell Southwest Confidentiality Policy.
The post holder demonstrates a positive
attitude to mental health, cultural awareness and attending to patients with
respect and courtesy and have the ability to challenge staff where the standard
falls below expectations.
The post holder will ensure that
professional boundaries are always adhered to.
Take a lead role in the collaboration of
the planning of individual care with patient, carer, other Teams and Services
always happens.
Must be confident in communicating with
people who are experiencing mental health difficulties when English is not
their first language.
Ability to negotiate with other service
providers to ensure all written information is accessible to all communities.
Enhancing quality of patient care and
promote initiatives to improvement the patient experience/services.
Ensure that the service delivers on both
local and national KPIs in relation to best practice.
Knowledge,
Training and Experience:
The post holder will be responsible for
updating their own practice, knowledge and skills within their sphere of
practice including Livewell Southwest mandatory training.
Knowledge of Policy and legislation.
Knowledge of Mental Health and Mental
Health service delivery and Capacity Act.
Adhere to Policy and Legislation
relating to Mental Health e.g. Mental Health Act 1983, Equal Opportunity Law,
Disability Discrimination Act, Human Rights Act, Care Programme Approach and
Clinical Governance.
Analytical
and Judgemental Skills:
Assisting the CMHT manager in the
interpreting of new guidance and develop methods of implementation.
The post holder will provide care and
other interventions in the setting most appropriate to meet the patients
needs.
Incorporate all guidance relating to the
Mental Capacity Act into all work.
Ability to match patient need with
available skills and resources
Ability to resolve inter-personal and
clinical issues in a decisive and impartial manner.
Problem solving, weighing up pros and
cons. Accessing relevant information to inform decisions and judgements.
Planning and
Organisational Skills:
To co-ordinate shifts ensuring
appropriate allocation of work with the available skill mix utilising
prioritisation, negotiation and delegation skills.
Support the acquisition of skills by
junior colleagues by positive role modelling and promoting a culture of
learning and personal development.
Regularly take a lead in meetings
ensuring all patients are reviewed and decisions and plans are put into action.
Liaise with other services to arrange
appointments that both meet patient and service demands. Making best use of available resources.
Physical
Skills:
Confidence to drive in an urban
environment.
Competent keyboard skills to enable
information to be inputted into S1 and eCPA.
Daily use of Microsoft word and excel packages. Must be able to use
Microsoft Outlook Calendar and manage emails in a timely fashion communicating
within the PCN and to external organisations within current policies and
guidelines.
Confidence to take on IT skills as
technology advances. Constant use of
laptop to record, monitor and plan patient care and to monitor and appraise
other team members work. This will
include supporting other staff in the acquisition of skills e.g. implementation
of the pilot of Electronic Daily Records.
Ability to take accurate physical
observations i.e. blood pressure, pulse and temperature.
Demonstrates safe clinical skills when
providing care for patients receiving specialist treatments e.g. commencement
of Clozaril in the community or supporting a patient through ECT treatment.
Home visits to multi-occupied buildings,
many without lifts.
Escorting patients out of their home
environment providing programmes of care and ongoing assessment of need. (E.g. Anxiety Management Programmes).
Ability to support patients meeting
their activities of daily living, e.g. working alongside a patient addressing
these needs.
Responsibilities:
Responsible for the supervision and
review of all ASR patients care within a specified geographical area. In addition to cover in the absence of the another
CMHT manager (this would be in collaboration with the other locality CMHT
Managers), providing supervision, line management and problem solving.
The post holder will act as a Care
Co-ordinator for a caseload of patients in accordance with the CPA Policy.
The post-holder will be responsible for
the roll-out of Link Working with 3rd sector agencies. Ensuring that standards are adhered to and
liaise with Primary Care colleagues to make sure that all Governance
arrangements are adhered to.
Take a lead in rapid crisis assessments
and the continuous assessment of patients on the caseload, monitoring
effectiveness of the treatments provided.
Responsible for the provision of specialised
advice and guidance relating to the management of risk and decision making
within the multi-disciplinary team and the wider health community.
Take a lead role in the identification
of risk and the formulation of risk management plans. Participate in risk
management meetings.
Regularly formulates specific
individualised care plans, implements and evaluates them alongside patients and
carers ensuring informed consent is obtained whilst working within the boundaries of the Mental Capacity Act. Plan aftercare, making appropriate agreed referrals
for longer-term treatment.
Collaborate with other Teams and
Services in the provision of patient care and ensure effective communication
for all patients within the geographical sphere of responsibility. To attend
reviews of their patients, in any part of the service.
Take a lead role in clinical
decision-making, including maintaining the safety of patients, staff and the
public in challenging, potentially violent situations.
Adhere to the Policy for the Safe Secure and Handling of medicines and local Home
Treatment protocols ensuring patients receive medication in a safe and timely
fashion.
Provide education to patients and carers
to enhance their knowledge therefore improving longer-term concordance.
Promote RECOVERY.
Provide education to patients and carers
in line with current policy and good practice guidelines.
Maintain high standards of clinical care
to patients through participation in regular audits.
Represent ASR at appropriate forums
nationally and locally.
Responsible for managing the
organisational interpretation policy, processes and management of bookings.
Responsibility
for Policy and Service Development:
Annual responsibility for the reviewing
and updating of all local policies and protocols. To provide up to date
information for staff at ASR Senior Staff Meetings.
Attend regularly the ASR Manager Meeting
which evaluates the Team and plans service developments.
Attend Acute Services Interface meeting
and participate in service development across the Directorate when required.
To attend Acute Pathway Interface
meetings to problem solve issues relating to the interface and tensions between
teams.
Implement all appropriate National, Livewell
Southwest and Local ASR policies and protocols.
Responsibility for financial and
physical resources:
Responsible for the ASR budget, meeting
regularly with management accountant to ensure allocated money is spent
effectively and within budget.
Responsible for the checking and
authorisation of time sheets and travel claims and forwarding on for payment.
This includes nursing and admin bank time sheets.
Ensure physical assets are monitored and
re-ordered on a monthly basis keep a watchful eye on expenditure ensuring costs
remain with allocated budget.
The post-holder will be an authorised to
make procurements using the ePROC system.
Responsible for the use of individual
corporate credit card making sure it is checked on-line each month and log completed
and sent off in time.
Responsibilities
for Human Resources:
Provide clinical supervision and line
management for a specified members of staff within the MDT this will include: a
number of Community Mental Health Nurses, Community Support Workers (Higher
Level) and Pre/Post registration students, Occupational Therapists and Social
Care staff.
To manage the individual CMHT on a
day-day basis ensuring that the skill-mix is appropriate to the needs of the
patients.
Have the ability to challenge staff
where the standards fall below expectation.
This will include poor performance issues.
Assist in education programmes provided
for junior staff.
Participate regularly in interview
panels.
The post holder will behave in a manner
that does not bring the Team or Livewell Southwest into disrepute.
