Health Care Support Worker

Job summary

We are looking for support workers, with previous hospital/community healthcare setting experience, to assist registered nurses and qualified therapists in delivering an excellent admission avoidance service as part of our Urgent Community Response Team (UCRT). This involves providing Clinical interventions, and support the recovery of patients who may be experiencing a health and social care crisis within their own home. The UCRT is a Community admission avoidance team providing a 7 day a week service in order to support patients to remain within their own homes where clinically safe to do so. The team provide a 2 hour response to referrals and offer specialist assessments and interventions as well as planning ongoing care for both health and social demand. Support workers will be expected:

  • To contribute to the assessment and the treatment of individual patients on the caseload and communicate any changes in health and care needs to the registered staff.
  • To communicate effectively with patients, relatives, carers and health care professionals to assist the registered staff in ensuring an integrated, holistic approach to patient care.

The post holder will work as part of an integrated multi-disciplinary team.

The post holder will require a valid driving license and access to a car as the service covers patients living in the Mid Essex area.

Main duties of the job

To manage daily workload efficiently and effectively, taking into account the needs and preferences of the patient.o To observe record and report to the Team Leader, any changes in a patient's condition or circumstances.o To be familiar with the steps to be taken in medical emergencies and act accordingly.o Work with the UCRT to support the delivery of holistic care and admission avoidance interventions.o To undertake through each patient's individual care plan, those duties delegated by the Team Leader and Case Manager, which the post holder has been trained and is competent and confident to perform.o To visit patients within their homes, at times, as directed by the registered staff or manager. Tasks may include:o Personal careo Monitoring of baseline physical observationso Timely Recognition and reporting of deteriorationo Blood glucose monitoringo Urinalysiso Phlebotomyo Male / Female catheterisationo Wound care managemento Full training and competencies will be provided to ensure that the post holder has the appropriate skills to undertake these duties.
