Job Overview:
This role supports individuals in making healthier lifestyle choices and improving wellbeing. You will conduct assessments to understand circumstances, identify barriers, and determine suitable support pathways. Through collaborative discussions, you provide advice and interventions on areas such as smoking cessation, weight management, emotional wellbeing, physical activity, and other lifestyle factors, empowering individuals to make informed health decisions.
You will coordinate access to local community, healthcare, and social services, providing clear signposting when direct support is unavailable, and maintain up-to-date knowledge of partner organisations to ensure seamless service delivery. Ongoing support includes follow-ups, monitoring progress, and addressing emerging challenges while maintaining engagement and accountability.
The role involves regulated activities, providing unsupervised support for young people and vulnerable adults, requiring strict adherence to safeguarding and professional standards. You will deliver evidence-based guidance, educate on healthy choices and social determinants of health, and keep accurate records using systems such as SystmOne and Priority Digital Health, ensuring all information is managed according to confidentiality and information governance standards.
Main duties of the job
Initial Assessments: Engage individuals via phone or digital channels to assess readiness for behavior change, identify barriers, and determine tailored support. Accurately document findings to guide interventions.
Support & Guidance: Collaborate with individuals to address health, wellbeing, and lifestyle needs. Deliver practical advice and interventions on smoking cessation, weight management, physical activity, and emotional wellbeing. Empower self-management and informed choices.
Service Coordination: Navigate clients through community, healthcare, and social services. Provide timely signposting and maintain strong knowledge of partner organisations for seamless support.
Ongoing Support: Conduct follow-ups to monitor progress, tackle emerging challenges, and maintain engagement through proactive guidance.
Regulated Activity: Support young people and vulnerable adults via the Single Point of Access, providing advice, guidance, and assessment while adhering to safeguarding and professional standards.
Evidence-Based Practice: Offer accurate, up-to-date information across health and lifestyle topics to promote long-term wellbeing.
Record-Keeping: Maintain confidential client records using SystmOne and Priority Digital Health in line with information governance standards.
