An exciting opportunity has arisen for an enthusiastic, motivated and experienced registered nurse to join our friendly, busy, yet rewarding team within the community of Mid Essex.
You will be working within the community to support the management of patients who have heart failure. Working in collaboration with our MSE partners to support a seamless patient pathway and to prevent hospital admissions and readmissions.
Main duties of the job
This post holder will responsible for and ensuring high quality, evidence based care for patients with the diagnosis of Heart Failure, their families and carers in the community. Managing patients at home, remotely or in the nurse led heart failure clinics.
The post holder will:
Titrate heart failure medications in accordance with the NICE guidelines
Exercise high levels of judgement, discretion and decision making in clinical care
Take responsibility for case management of patients with heart failure in community settings, including venepuncture, diuretic therapy, and end of life care and MDT
Plan and provide treatment in collaboration with others as appropriate
