Job summary
An opportunity has arisen for a highly motivated and experienced Speech & Language Therapist (SLT) to join Provide CIC and work as a key member of a small team in the delivery of high dose therapeutic interventions to people with aphasia across the Mid and South Essex (MSE) region. This new service is funded as a 'catalyst project' via the national Stroke Quality Improvement for Rehabilitation (SQUIRE) programme and will run for one year.
The project seeks to support and augment the management of people with aphasia (PWA) in the first six months after stroke and aims to ensure early access to high quality education for PWA and their families, guidance for teams working with PWA across Provide and MSE partner organisations (EPUT, NELFT & MSEFT) and delivery of high dose, high intensity in-person therapy to small groups of PWA during the 3-6 month phase of recovery close to where they live.
Additionally, the service aims to direct PWA and their families to other appropriate sources of support and communication practice, aims to offer remote therapy interventions alongside therapy from other teams in the integrated stroke service and aims to coordinate self-directed therapy via tablet-based apps. Performance and outcome information for PWA will be collected at set intervals and maintained in a secure database.
Main duties of the job
To jointly deliver high-dose interventions to people with aphasia (PWA) across the Mid and South Essex (MSE) region.
To contribute to the design and implementation of service user engagement and documentation systems.
To liaise closely with all MSE acute SLT teams to ensure early registration and early delivery of support of all people with newly acquired aphasia and their families.
To liaise closely with all MSE Integrated Community Stroke Service (ICSS) teams to offer support and advice regarding treatment and to provide additional support and treatment opportunities to PWA.
To contribute to the design, implementation and maintenance of a database for PWA in the MSE region.
To advise on complex cases using highly specialist level clinical reasoning, critical thinking, reflection and risk analysis to best support PWA and their families in challenging circumstances.
To support the collection of all appropriate contact and outcome data for the Stroke Sentinel National Audit Programme (SSNAP).
To engage with key stakeholders to progress the planning of the ICSS.
To provide regular high quality supervision colleagues and to act as clinical educator to University students.
To contribute towards and support reporting of service achievements to the wider aphasiology and stroke community.

This is a highly specialist post working in a team to deliver an augmentative service for people with newly acquired aphasia post-stroke and their families.
- The post holder will support colleagues in other MSE ICSS teams with highly specialist assessment, diagnosis and treatment decision making and will advise ICSS teams re. communication recovery promotion.
- The post requires highly specialist level knowledge of acquired communication disorders (i.e. agnosia, anosognosia, apraxia, alexia, agraphia, attentional impairment, visual or auditory perceptual impairment, memory dysfunction, executive dysfunction, emotional processing impairment, pragmatic impairment and dysarthria) that can co-present alongside language processing disorder (aphasia).
- The post holder will demonstrate highly specialist skills in simultaneously monitoring auditory, visual and kinaesthetic aspects of communicative interactions, responding in a manner that elicits increased participation and maximises effective communication via the use of adaptive strategies.
- The post holder will be responsible for identifying clinical priorities and the implementation of client-centred management programmes that seek to establish a consistent approach across all members of the care team, family and wider system.
- The post requires a good working knowledge of preferred aphasia therapy apps and the ability to calibrate, monitor and adjust individuals self-directed exercise profiles.
- The post holder will demonstrate highly specialist skills in supporting the acquisition of effective multi-modal communication (encouraging use of the environment/props, gesture, drawing, writing and noises/sound effects etc.) to supplement verbal output and assist the effective conveyance of a message.
- The post holder will be required to evaluate and advise on the potential of augmentative or compensatory aids to assist people with complex communication disorders to function more effectively.
- The post holder will demonstrate highly specialist skills in supporting people with aphasia to take as much of a lead in their recovery as possible supporting their identification of personally relevant goals and assisting their recognition of progress.
- The post holder will demonstrate highly specialist level abilities in the motivation of sustained cooperation with treatment initiatives where reduced insight, cognitive function, confidence and/or mood disorder might challenge engagement.
- The post holder will hold responsibility for providing defined interventions to PWA and will use advanced clinical reasoning, critical thinking, reflection and analysis to deliver a highly effective client centred, goal-orientated rehabilitation programme.
- The post holder will rigorously adhere to the available evidence base in the design and implementation of education and treatment approaches/practice.
