Job summary

We are looking for bank Health Care Support Workers to assist qualified therapists, registered nurses & paramedics in delivering an excellent admission avoidance service as part of our Urgent Community Response Team (UCRT).

As this post involves community work, successful candidates must have a full driving license and access to a car.

Main duties of the job

The UCRT is a Community admission avoidance team providing a 7 day a week service in order to support patients to remain within their own homes where clinically safe to do so. The team provide a 2 hour response to referrals and offer specialist assessments and interventions as well as planning ongoing care for both health and social demand
