In this dynamic role, you will work directly with patients, carers, and community health staff to break down barriers to digital access. Whether you are helping someone register for the NHS App for the first time or running a digital skills workshop, every day brings new opportunities to empower the community you will be working with.

You will collaborate with Nottingham CityCare's clinical services, GP practices, voluntary sector partners, and patient groups to reduce health inequalities and promote digital inclusion. Your mission: to ensure individuals, regardless of background or ability, can confidently access and benefit from local health services online.

Main duties of the job

  • You'll thrive on engaging with a diverse range of people, building trust, and making digital health feel accessible and welcoming.
  • From one-to-one support to group sessions and outreach in community venues, your work will directly improve lives and help close the digital divide.

Job Purpose

Nottingham and Nottinghamshire Integrated Care System (ICS) Digital Inclusion Workstream is working to improve digital health inclusion for the local population and workforce by embedding Digital Inclusion Co-ordinators within Primary Care Networks (PCNs) and local organisations.

The Digital Inclusion Co-ordinator for Nottingham CityCare will provide proactive digital skills support to staff and members of the public within Nottingham Citys diverse communities. This will include supporting our services to reduce digital health inequalities and increase the uptake of digital services. The postholder will support the ICS vision of utilising the NHS App as the digital front door by actively engaging with patients, citizens and staff to improve their awareness and access to digital healthcare services.

The Digital Inclusion Co-ordinator will be based with Nottingham CityCare but will also be a pivotal member of a wider ICS digital inclusion team, delivering innovative support activities through outreach work, promoting the benefits of digital healthcare tools and acting as first point of contact for digital services support.

The postholder will ensure the provision of information as required, for stakeholders both internal and external. This will involve presenting to a range of audiences, targeted work focusing on the clinical priorities of Nottingham CityCare, utilising relevant clinical systems and updating reports / documents as needed. The Digital Inclusion Co-ordinator will undertake reporting, tracking and analysis to understand the impact of the digital inclusion support schemes implemented and adapt to service need.

Key Responsibilities

  • Responsible for coordinating and delivering digital inclusion support activities at planning, development and delivery stages to accelerate and maximise digital inclusion across Nottingham CityCares services and communities.
  • Build and maintain networks through engagement with practice teams and the wider community. Utilise influencing and communication skills to educate others on the benefits of digital services, whilst recognising exclusion barriers and challenges.
  • Upskill and improve confidence in using digital services through tailored support and training of members of the public and staff. Coordinate training opportunities for Nottingham CityCares staff and local third sector organisations.
  • Utilise strong communication skills to deliver presentations to a wide range of audiences.
  • Contribute to the positive promotion of digital health services available to members of the public (NHS App and other / integrated services) for improved management of health, care and wellbeing.
  • Work with local colleagues to identify key cohort groups to target activity, reducing the barriers to digital inclusion for those that experience the greatest health inequalities and ensuring positive impact on patient outcomes.
  • Responsible for monitoring progress against agreed objectives and targets, ensuring that reporting takes place. Where necessary evaluate own work and impact relating to specific activities undertaken and use this information to develop services as appropriate.
  • Develop communication materials that can be included within newsletters, social media posts and practice websites, showcasing the benefits of digital healthcare services and the impact of the roles work.
  • Responsible for supporting Nottingham CityCare in having health literate information on relevant websites about digital health services and the support available through the Co-ordinators own role, ensuring it is accurate and kept up to date.
  • Complete mandatory / other training to support the knowledge of the post-holder. Act in accordance with the policies and procedures of the organisation.
  • Work closely alongside other staff on wider digital transformation projects aligned to Nottingham CityCares / ICS priorities.
  • Work in partnership with a vast network, including: the local digital inclusion workstream, local digital health champions network, CVS Organisations, national NHS App team and NHS App Ambassadors by sharing and acquiring information, knowledge and opportunities.
