Job summary

Nottingham CityCare Partnership is a dynamic organisation which provides a broad range of services to improve outcomes for children, young people and their families within a diverse multi-cultural city and we have an exciting opportunity to join our Childrens Public Health 0-19 Nursing Service in the role of Public Health Practitioner.

The Public Health Practitioner will work across Nottingham City supporting the delivery of public health and health promotion interventions as part of the Healthy Child Programme for babies, children, young people and families which includes Group and 1:1 work with families in a variety of community and virtual settings

You will participate in the development and rollout of training to our workforce and work alongside the Childrens Public Health 0-19 Nursing skill mix teams to support delivery of high quality public health services to children, young people and their families. This willinclude an annual Public Health programme including Groups and Health promotion and awareness raising across all of our Care Delivery Groups embedding the NHS Calendarof National campaigns, coordinating our approach with local focus.

Main duties of the job

Support delivery of public health interventions as part of the Healthy Child Programme for babies, children, young people and families.

Support the development of training and deliver/co facilitate training.

Undertake programmes of care to improve public health, including but not limited to promoting healthier diet and nutrition intake and physical exercise goals by supporting parents with nutrition, physical activity and sleep management support, in a variety of settings.

Deliver training around health promotion and programmes of care using virtual and face to face delivery methods.

Work with partner agencies to ensure our Public Health promotion matches the current Public Health agendas.

As part of the Public Health team search for public health needs, emerging issues for children and families in Nottingham City.

Contributes to the development of Childrens Public Health 0-19 Nursing Service annual public health plan for service delivery including planning and distribution of promotional materials, displays, literature, and resources.

Support the coordination of public health and health improvement campaigns.

Your practice must adhere to clinical guidelines, policies, and Standard Operating Procedures (SOP) both on a local and national level and you must demonstrate Nottingham CityCare Partnership CIC values and behaviours in all you do.

Dimensions

To work under the direction and leadership of a Clinical Service Manager/Lead/Specialist, Community Public Health Nurses and other team members and be integral to the 0-19 Childrens Public Health Nursing Service.

Work alongside the Childrens Public Health 0 -19 Nursing skill mix teams to support delivery of high quality public health services to children, young people and their families in Nottingham City.

Deliver health promotion interventions to children aged 0 -19 years old and their families in a variety of community and virtual settings.

Work autonomously to organise and complete delegated work effectively, seeking advice where necessary.

Undertake other duties as may be required for maintenance of an effective and efficient service.

Work with partner agencies to ensure our Public Health promotion matches the current Public Health agendas.

Day to day supervision; proactive co-ordination of staff to deliver group activities or key contacts across the city.

Core service hours are 8.30am to 5pm, Monday to Friday excluding bank holidays

Key Responsibilities

Quality and performance

  • Undertake public health interventions supporting parents and children, using behaviour change strategies to bring about readiness to change, essential for healthier lifestyle changes, including but not limited to; a healthy diet, nutrition and physical exercise, immunisation uptake, oral health, sleep, behaviour and childhood development.
  • Carry out assessments, including growth measurements, to enable goal setting, in partnership with children and families, to be able to meet specific, measurable, realistic and time framed outcomes as part of the Programmes of Care and childhood development as per the Healthy Child Programme (DoH 2009).
  • Undertake programmes of care to improve public health, including but not limited to promoting healthier diet and nutrition intake and physical exercise goals by supporting parents with nutrition, physical activity and sleep management support, in a variety of settings.
  • Deliver training around health promotion and programmes of care using virtual and face to face delivery methods.
  • Create and design public health promotional resources to deliver training and education using desk top publisher and other means of IT software for resource production including participation in website and social media platform materials eg videos and podcasts and display board layouts.
  • Be able to plan, organise and facilitate groups to deliver health promotion messages e.g. first foods, balancing milk to meals, oral health, healthy weight, physical activity, and others, using virtual and face to face delivery methods.
  • Day to day co-ordination of staff, ensuring cover for groups and health promotion work.
  • Promotion of Healthy Start and use of Vitamin D for those pregnant and breastfeeding and for babies and children.
  • Provide routine information to children young people and families which require persuasive and motivational interviewing skills, particularly when there are barriers to understanding and recognition of a family that is not ready to change.
  • Support the planning and implementation of the annual National Child Measurement Programme.
  • Work collaboratively with caseload holders to ensure any safeguarding concerns are discussed and acted on.
  • Coordinate focus groups and surveys to support co production and consistently audit and evaluate the required service delivery using skills for analysing health improvement materials.
  • Receive information and feedback from clients who may have difficulty conveying their views and ensure provision is made to facilitate their ability to achieve this e.g. when English is not the first language.
  • Undertake targeted health promotion initiatives in a variety of settings for displays and health promotion focused events.
  • Ability to contribute towards achieving key performance targets, preparing and entering health improvement data that will then inform service improvements and delivery.

  • Contribute to processes to maintain stock control, ordering and distribution of resources for the Childrens Public Health 0-19 Nursing Service.
  • Support the use of electronic communication for sharing ideas, content, information and messages. Digital media may include text messages, emails, internet based messaging services and web chats.

  • Contribute to the promotion and uptake of enhanced offers/incentive schemes to children and families e.g. imagination library.
