Job summary

Are you a qualified Physiotherapist looking for an exciting new opportunity?

As part of Nottingham CityCare Partnerships Integrated Care Homes Service (ICHS), you will work within the Dementia Outreach and Therapy teams to assess care home residents with dementia and create treatment plans. Your role will focus on falls management, mobility, and manual handling, supporting independence and aiming to reduce hospital admissions.

ICHS aim is to improve residents and staff experience by delivering high-quality, multidisciplinary care in an integrated model, with referrals prioritised to meet individual needs. Caseloads are primarily adults with dementia in care homes. Applicants should demonstrate strong Physiotherapy skills and experience supporting people living with dementia, ideally within community settings.

Professional development is actively encouraged, with opportunities to collaborate with specialist services and access ongoing education. Team members benefit from regular therapy meetings, peer support, and supervision.

The Dementia Outreach and Therapy teams play a vital role in optimising wellbeing through person-centred rehabilitation. By joining, you will gain valuable experience managing complex cases and contribute to improving the quality of life for vulnerable adults.

Main duties of the job

This Physiotherapy role is part of the Dementia Outreach (DOT) element of the Integrated Care Home Service, which includes dementia specialist therapists, RMNs, and Assistant Practitioners. DOT therapists work alongside physical therapy clinicians as an integrated team, supported by a therapy lead. Referrals are triaged and assigned to the most appropriate therapist.

The DOT Physiotherapist must demonstrate advanced clinical knowledge and experience in dementia care. They will complete holistic assessments, prescribe tailored therapy programmes, and manage their own caseload. The role also includes supervising Assistant Practitioners and acting as a resource for the wider service.

As an integrated service, collaborative working is central. ICHS Therapy team and wider therapy meetings are held monthly, for therapists from across CityCare to share expertise and develop innovative practice. The Physiotherapist will contribute to service evaluation, audits, meetings, and working groups, representing the service where required.

The team supports delivery of the Enhanced Care in Care Homes Framework, providing training and education to care home staff, empowering them to deliver high-quality care. The role also involves developing joint handovers in care homes to enable proactive support and prevent avoidable hospital admissions.

The DOT element of the service is currently undergoing an ICS-wide service review.

  • To be responsible for continuing the ongoing development of high standards of clinical practice and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution to the work of a multi-disciplinary team
  • To advise and support care home staff in maintaining a residents wellbeing and engagement despite.
  • To manage/care coordinate a caseload of patients, and will provide advice, education and teaching sessions to providers of residential/nursing care.

Dimensions

Key Responsibilities

Clinical

  • To be professionally and legally responsible and accountable for all aspects of your work, including the management of clients in your care. To ensure a high standard of clinical care for the clients under physiotherapy management, and support more junior staff to do likewise.
  • To determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each client and/or carer. This may include dealing with complex and conflicting opinions within the multi-disciplinary multi-agency team.
  • To undertake the comprehensive assessment of clients, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non-standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care.
  • To set physiotherapy rehabilitation goals, carry out treatment interventions.
  • To accept clinical responsibility for a designated caseload of clients and to organise this effectively with regard to clinical priorities and use of time.
  • To provide highly specialised advice, teaching and training to other members of the Integrated Team regarding the physiotherapy management of clients with problems related to rehabilitation/mobility/ageing.
  • To train, supervise and manage more junior staff, support staff and students. This will include the use of formal appraisal documentation and accreditation as a Physiotherapy Clinical Educator.
