Job summary

An opportunity has arisen for an enthusiastic, conscientious, passionate and motivated physiotherapist with a passion for working in the community and a keen interest in the holistic working model, to join the Integrated Reablement Team, incorporating the Urgent Community Response Service, operating across Nottingham City.

The service consists of a multi-disciplinary team (MDT) that includes Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Registered Medicine Technician, Assistant Practitioners, Rehabilitation Support Workers and administrative support. The aim of the MDT is to promote, improve and maintain independence through rehabilitation and crisis management with citizens in their own home or within a short term care setting to prevent unnecessary hospital admissions and support hospital discharges.

You should have an understanding of Health and Social care provision and the ability to work closely and collaboratively with partner organisations and other stakeholders. We respond to urgent referrals from a variety of sources such as Ambulance services, GPs, Social Services etc.

You will be supported to hone your assessment and intervention skills in order to plan, develop and evaluate goal focussed rehabilitation and reablement programmes, as well as complex discharge planning experience.

CityCare will offer you an excellent Preceptorship Programme to support and develop newly registered AHPs as well as a bespoke local induction programme

Main duties of the job

To work as an autonomous practitioner and undertake comprehensive assessments to identify the appropriate service for the clients referred.

To carry out profession specific, and holistic, assessments and evaluation as required.

To assist the multi-disciplinary team to plan and develop a goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs either in a community bed or in their own homes.

To be responsible for continuing the ongoing development of high standards of clinical practice with evidence based focus.

To work as required within the Urgent Community Response and Intergrated reablement team, responding to health and social care referrals with the aim of preventing admission to hospital or a care home, facilitating discharges from community beds and acute settings and promote independence.

Able to fulfil physical demands of role, including manual handling applications.

Will be required to travel city-wide for client visits, meetings and training.

To participate in rehabilitation treatment programmes with the MDT

To carry out training of other members of staff.

Support senior colleagues with the facilitation of student placements.

To be professionally and legally responsible and accountable for all aspects of your work, including the management of citizens in your care. To ensure a high standard of clinical care for the citizens under physiotherapy management, and support more junior staff to do likewise.

To determine by assessment and liaison with other relevant agencies the advice, education and treatment requirements of each citizen and or carer. This may include dealing with complex and conflicting opinions within the multi-disciplinary multi-agency team.

To undertake the comprehensive assessment of citizens, including those with a complex and/or chronic presentation using investigative and analytical skills through standardised and non standardised assessments. To formulate individualised management and treatment plans, using clinical reasoning and using a wide range of treatment skills and options to devise a specialised programme of care.

To set physiotherapy rehabilitation goals, carry out treatment interventions and/or home assessment visits (often as the sole health professional)

To accept clinical responsibility for a designated caseload of citizens and to organise this effectively with regard to clinical priorities and use of time

To provide highly specialised advice, teaching and training to other members of the team regarding the physiotherapy management of citizens with problems related to rehabilitation/mobility/ageing.

To train, supervise and manage more junior staff, support staff and students. This will include the use of formal appraisal documentation and accreditation as a Physiotherapy Clinical Educator. To supervise, educate and assess the performance of undergraduate Physiotherapy students as required to ensure the standard of practice and teaching meets the standards set by the degree level qualification

To have the ability to gain valid informed consent and to identify through assessment citizens who lack capacity and work within a legal framework with citizens who lack the capacity to consent to treatment.

To work as a lone practitioner with supervision from band 6 clinicians. The Team Manager will also provide supervision.

To provide spontaneous and planned advice, teaching and instruction to citizens, carers and other professionals to promote understanding of the aims of physiotherapy and to ensure a consistent approach to citizen care.

To assess, advise and where appropriate provide mobility aids including teaching citizens / carers in their use.

Organisational

To participate in the development of good practice for Physiotherapists within Nottingham CityCare Partnership under the guidance of the Professional Lead Physiotherapist

To demonstrate a commitment to evidence-based practice.

To be responsible for own Continuing Professional Development under the supervision of the band 6 physiotherapist keeping abreast of new trends and developments, incorporating them into your work as necessary. Attend relevant conferences and internal/external training courses. Support more junior staff to do likewise.

To maintain a high level of physiotherapy standards and work to the Physiotherapy Code of Professional Conduct.

To actively participate in the services training strategy, including in-service training, and multi-agency training. To participate by attendance and presentation at staff meetings, tutorials and training sessions.

To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to the work situation.

To assist with development and participation in the teams audit projects, evidence based practice projects and outcome measures, either individually or with other senior therapists. To make recommendations for change.
