Assistant Practitioner (Virtual Wards)

Job summary

Nottingham City Care is looking for Assistant Practitioners to join our exciting new virtual ward team within Nottingham City and Nottinghamshire.

Our Assistant Practitioners will be an integral part of our virtual ward providing clinical expertise and knowledge to our patients and fellow clinicians. They will be providing care and support for to patients for up to 14 days that have an acute illness that can be treated in their own homes. The aim is to reduce hospital admissions. The Assistant Practitioners will also be providing care and support to patients that will be discharged from hospital that are medical unwell but stable and allowing them to be discharged back to their own homes earlier reducing the number of days that they are staying in hospital.

The Assistant Practitioners will be working shifts, 30 hours (Monday - Sunday), between 8am and 4pm, 10am and 6pm and 12noon and 8pm. Based at The Mary Potter Centre, but visits across the city.

Main duties of the job

You will be part of the wider multi-disciplinary team, working alongside senior nurses, assistant practitioners, consultants, OTs, Pharmacists, secondary care and social services.

  • Carrying out clinical observations e.g BP, temp etc
  • Following treatment/care plans
  • Ensuring the provision of continuity of care, including follow-up visits.
  • Assessing and evaluating, with patients, the effectiveness of the treatment and care provided and make changes as needed.
  • Provide support to fellow clinicians
  • Making sure that each patient's treatment and care is based on best practice.
  • Seeing patients in their own home.
  • Working as part of multi-professional and multi-agency teams.

To provide assessment and treatment intervention to people transferred from secondary care with an acute respiratory illness to the virtual ward within the Nottingham City and Nottinghamshire Healthcare. Facilitating the early discharge from hospital and caring for people in their own homes.
  • To support the work of the clinicians, providing assessments of needs, and working across professional boundaries and within legislation, policies and procedures and core values.
  • To maintain records as an autonomous practitioner working with supervision

Dimensions

  • You will be working in patients own home or residential home following treatment plans and monitoring the patients condition/progress.
  • Undertake clinical observations such as BP, temp, oxygen saturations etc
  • You will participate actively in the services training strategy, including in-service training and multi-agency training.
  • Working in conjunction with and under supervision of senior clinical professionals.
  • Able to fulfil physical demands of role, including manual handling applications.

  • To undertake individual treatment and interventions with clients under the supervision of qualified staff.

  • To accept responsibility for designated caseload, working independently taking an active role in promoting independence and meeting the needs of the service user.
  • To promote wellbeing, safe mobility, transfers and effective communication. To provide instruction, advice and intervention to service users and carers in line with the multi-disciplinary team guidelines.
  • To assess, provide, fit and demonstrate use of a wide range of equipment to promote independence.
  • To assess for and commission a range of adaptations in line with the multi-disciplinary team guidelines.
  • To attend and contribute to team meetings and case discussions using effective communication.
