Job summary
We are excited to be adding a dedicated Diabetes Specialist Nurse to our established, forward thinking, Integrated Diabetes Team. We are seeking an individual who is motivated, a team player and can relate with CityCares values and behaviours to form part of our high performing, highly skilled team.
The successful candidate will play a crucial clinical role in the person-centred assessment, planning, and coordination of care and support for people in Nottingham City with diabetes.
The aim of this role is to promote engagement with appropriate services, contribute to more effective specialist management, and prevent potential negative health outcomes of diabetes, including avoiding unnecessary diabetes-related hospital admissions.
As part of this recruitment process the service will consider a B6 to B7 development opportunity in line with CityCares progressive culture and commitment to our strategic aim to provide rewarding working lives and careers.
Main duties of the job
Key roles of this opportunity are to:
Work closely with GPs, long-term conditions case managers, and other primary/community care staff to support people with diabetes in managing their condition more effectively.
Collaborate with Diabetes Teams in secondary care to ensure appropriate patients are identified for referral into community-based services.
Coordinate care of patients between primary and secondary care settings, acting as a specialist resource.
Contribute to the development of the Diabetes Specialist Nurse role to continuously improve the treatment and support for people with diabetes and their carers.
Support and maintain a clinically managed network of multidisciplinary professionals, promoting consistent evidence-based practice to ensure efficient and effective use of available resources.
Participate in diabetes network working groups, particularly those focusing on clinical standards, pathways, and outcomes
