Job summary
The Continuing Care Service for Greater Nottingham
is looking for an experienced nurse to join the team as a Band 7 Continuing
Care Case Manager. The post-holder will provide clinical input into the
assessment and case management for CHC eligible patients and will ensure that
patient care is planned, managed effectively and successfully delivered. There
is a responsibility for facilitating CHC assessments and reviews against the
national framework, within the agreed timescales. The successful candidate will
promote high standards of communication and liaison with patients, their
representatives and Health and Social Care colleagues. Candidates should be
experienced Registered Nurses with a working knowledge of the National
Framework for NHS Continuing Healthcare and associated processes.
You will be part of a team of experienced health
professionals including Mental Health , Learning Disability and childrens
specialties. There is an opportunity to work autonomously and flexibly as you
will be responsible for your own caseload. The role will also allow hybrid
working.
Main duties of the job
The Case Manager will be directly involved in
developing the service, implementing new ways of working, and commissioning of
health care packages at home and other community settings to prevent hospital
admissions.
In addition they will:
- Help facilitate early discharge from
hospital and prevent re admissions.
- Ensure that the
health care needs of adults eligible for Continuing Healthcare are identified
and met in accordance with the National Framework for NHS Continuing Healthcare
and Funded Nursing Care (July 2022).
- Be required to hold a
caseload of complex patients and will need to complete case manager reviews and
eligibility reviews and be able to commission a high standard of care for adults
with complex health needs and life threatening or chronic illness. The
caseload will include patients with physical health needs, learning disability
and mental health needs.
- Liaise directly with care
providers and negotiate effective care
packages with adults and their families/carers, which meet the requirements of
the individual.
- Able to implement and assist in the
coordination of multi-agency packages of care with health, social care and
education and support the continuing care assessor in day-to-day management of
the service.

- To deputise for Service Head in attending Joint Continuing Care
Panel in providing specialist clinical advice for the eligibility for
Continuing Healthcare.
- To provide specialist clinical advice to the Triage panel for all
patients accessing the Continuing Care service.
- To be responsible for the promotion of best practice that ensures
the health, safety and security of self, patients and colleagues in accordance
