Job summary
An exciting opportunity has arisen for a Band 6 Integrated Primary Care Cardiac Nurse to join Nottingham CityCares Integrated Primary Care Cardiac Service on a permanent contract working 37.5 hours per week.
The service is forward thinking, dynamic and provides evidence-based assessments and follow ups for individuals requiring urgent and routine symptom management and provision of various therapies in the community as well as rehabilitation and education in both home and clinic settings.
We are seeking an enthusiastic, motivated, and conscientious professional with cardiac experience. The team will provide the successful applicant with the opportunity for postgraduate and specialist training to enable them to build on their current cardiac knowledge enabling them to support patients to be independent in their community. You will undertake routine clinical management of patients with cardiac needs in an integrated manner within the service pathway. Within the wider service you will be involved in audit, development of practice and take an active role in case management.
You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritise tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in decision making. You must be willing to undertake specialist training as required. You will need to hold a valid driving license and have access to a vehicle for this role.
Main duties of the job
To provide holistic and individualised care to patients and carers within the Primary Care Cardiac Service.
To promote the health of patients in the community who are at risk from and who have been diagnosed with cardiovascular disease.
To be responsible for a designated caseload of patients.
To plan, deliver and evaluate individual cardiac rehabilitation packages, liaising with and referring on appropriately to all other relevant agencies and services. To work within the service to provide advice and care to patients within the Primary care cardiac team using a collaborative approach with colleagues and the wider MDT.
To take a lead role in line management within the service.
To work collaboratively and across organisational boundaries to provide evidence-based practice and holistic care to patients in a variety of settings.
