Dimensions

To provide holistic and individualised care to patients and carers within the Primary Care Cardiac Service.

To promote the health of patients in the community who are at risk from and who have been diagnosed with cardiovascular disease.

To be responsible for a designated caseload of patients.

To plan, deliver and evaluate individual cardiac rehabilitation packages, liaising with and referring on appropriately to all other relevant agencies and services. To work within the service to provide advice and care to patients within the Primary care cardiac team using a collaborative approach with colleagues and the wider MDT.

To take a lead role in line management within the service.

To work collaboratively and across organizational boundaries to provide evidence-based practice and holistic care to patients in a variety of settings.

Key Responsibilities

Assess health and social needs, develop, implement and evaluate individualised programmes of care to promote the health of patients in the community who would benefit from cardiac rehabilitation and secondary prevention.

Case manage patients holistically which may include patients with multiple cardiac issues.

Work as an autonomous practitioner in line with professional standards

Be a non-medical prescriber or be willing to undertake course within 18 months.

Work in collaboration with primary care teams, secondary care teams and other professions

Support effective team working

Develop and maintain close working relationships with other statutory bodies, community groups and voluntary agencies with the aim of reducing the incidence of cardiovascular disease and improving access to services in high risk groups within local communities.

Design and deliver training packages and awareness raising sessions for a variety of health care professionals and community groups with the aim of improving service delivery, enhancing local knowledge and skills.

Develop and contribute to the development of new services to meet the needs of patients with Cardiovascular Disease, specifically heart disease.

Actively promote patient and public involvement in the planning of individual packages of care and service design and delivery.

Maintain professional skills and knowledge to ensure expert practice within clinical area.

Initiate and /or participate in relevant research, audit and evaluation activities as appropriate.

Maintain contemporaneous records and undertake administrative duties as necessary

Participate in supervision activities

Participate in the interview process and recruitment of team members.

Undertake any other duties that may be reasonably required within the remit of the band 7 grade.

Identify health and social care needs, liaise with other multi-disciplinary team members, make appropriate referrals and communicate sensitive information to a vulnerable client group.

Raise awareness and profile of Cardiovascular Disease.

Manage complex situations with a vulnerable client group. Respond rapidly to the constantly changing environment and client needs. To continually risk assess situations.

Plan and take part in health promotion projects.

Assist in setting maintaining, monitoring and evaluating standards of care within the service.

Use specialist knowledge to influence strategy and improve provision throughout Nottingham, in health, local government and voluntary sectors.

Maintain a high standard of care to patients and carers.

Maintain a professional image as outlined in NMC code of conduct.

Act as a mentor to a variety of nursing students

Participate in the development of other team members.

Ensure that confidentiality and data protection is adhered to at all times

LIAISON

Establish and maintain effective channels of communication throughout the multidisciplinary team.

Liaise with Acute and Primary Care staff and integrated teams in order to ensure the patients holistic needs are met. This could be in a nursing, advisory or teaching capacity.

Take on role of lead responsible clinician, advise on any problems occurring whilst the patient is being visited at home and advise on how to move forward regarding the issue.

Be involved with wider cardiac rehabilitation strategies across the system

EDUCATION

Organize and participate in appropriate seminars and study days, acting as a specialist resource.

Contribute to the education and support of other clinicians within the primary care setting.

Educate, support and counsel patients with cardiac problems including their carers and family members whenever possible.

Contribute to raising awareness of the role of the Primary Care Cardiac Service including education and training for patients and other health care professionals.

Attend all senior meetings and team meetings

Play an active role in case management and lead and support on updating and developing new SOPs.

MANAGERIAL

To ensure that policies and protocols related to the clinical role are available to all staff and assist in their updating.

To ensure all documentation is accurately maintained.

Take responsibility for line management within the service

Take a lead role for identified aspects of the service

CLINICAL GOVERNANCE

Assist with the investigation and reporting of all incidents relating to the service, ensuring appropriate action is taken.

Ensure an approach to care, which recognizes the unique needs and views of cardiac patients and their carers and which places the patient at the centre of all activities.

RESEARCH AND AUDIT

To promote and display evidence of developments in nursing and related science research which enhance the level of professional knowledge and skill.

To participate and contribute towards any related research programmes that have been formally agreed within the limits of time and funding.

Ensure that evidence-based findings are translated into practice.

To collect and utilize data to enable monitoring and evaluation of the service, which will provide evidence of the value and effectiveness of the service.

Support clinical audit as a means of enhancing clinical practice and being receptive to the audit plans of others.

Person Specification

Qualifications

Essential

• RN qualification on part 1 of the register

Desirable

• Post registration qualifications relevant to community specialist role, health promotion, cardiac rehabilitation or cardiology

Experience

Essential

• A Minimum of 2 years experience where the following skills have been developed:
• Holistic care of people in a community setting, within cardiac rehabilitation or within a cardiology setting
• Experience of staff management.

Desirable

• Experience in a primary care setting
• Experience in a cardiology/cardiac rehabilitation setting
• Experience delivering health promotion/facilitating lifestyle change

Special Requirements

• Ability to be flexible over hours worked within contracted hours to meet the needs of the service
