Job summary

An exciting role is available for a registered clinician to join the Nottingham City and Nottinghamshire Transfer of Care Hub (ToCH)

The ToCH is a System-level hub where all relevant services (acute, community, primary care, social care, housing and voluntary) work together to triage, coordinate care and support for people who need it to enable timely discharge, recovery, reablement and rehabilitation. The ToCH is co-located within the acute hospital and holds responsibility for developing timely and person-centred discharge plans for people based on the principle of home first.

The ToCH aims to improve the journey of the person leaving the hospital with fewer hand-offs, resulting in no duplication of assessments, more accurate assessments undertaken in surroundings that are familiar to them rather than in a hospital environment, less time in hospital and therefore less time away from their own bed. This reduces the likelihood of deconditioning, infection, confusion/disorientation, institutionalisation, or increased dependence.

System partners are present in the ToCH to triage discharge to assess (D2A) referrals, identifying the most appropriate discharge pathway for the presenting needs and collaborating to meet those needs in the most time efficient way, aiming for discharge within 24 hours of the person becoming medically safe.

An understanding of Health and Social care provision and ability to work closely with partner organisations and other stakeholders is essential.

Main duties of the job

This role, as part of the MDT, will evaluate and discuss the D2A information and make decisions about the best discharge destination for the individual, facilitating the admission where appropriateby answering any queries that the receiving service/destination may have to best meet their needs.

The aim is that nobody must make (or have made for them) lifetime decisions in hospital and people leaving hospital should get the right care in the right place at the right time. The ToCH multi-disciplinary team that this role is a part of, support this aim through triage, discussions and knowledge of system wide provision.

People who can be supported to return home on Pathway 1 will be triaged throughout the day and arrangements made to book the required care and therapy, those whose needs are more complex and may require Pathways 2/3 will be discussed to ensure that an agreement is reached to facilitate discharge on the most appropriate pathway.

You will need excellent assessment and intervention skills, as well as complex discharge planning experience, demonstrate high standards of evidence based clinical practice, have excellent organisation, interpersonal and communication skills and should be able to prioritise your workload, manage your time effectively, and work flexibly within the team.

This post sits within the ToCH but is line managed and supported by Nottingham CityCares Community Traige Hub team. This role will be considered as a job share.
