Nottingham City Care is looking for an innovative & dynamic Community Matron to join our new virtual ward team within Nottingham City.
Virtual wards are a national initiative and defined as "A Virtual Ward is a safe and efficient alternative to NHS bedded care which is enabled by technology" NHS England and NHS Improvement.
Nottinghamshire Integrated Care System, aim to deliver this through a Nottinghamshire wide provider collaborative, incorporating Sherwood Forest Hospitals, Nottingham University Hospitals, Doncaster & Bassetlaw Hospital, Nottinghamshire Healthcare and City Care to deliver an ambition of 400 - 500 beds across the Nottinghamshire by Dec 2023.
Our Community Matrons will be an integral part of our virtual ward providing clinical expertise and knowledge to our patients and fellow clinicians. They will be providing care and support for to patients for up to 14 days that have an acute illness that can be treated in their own homes. The aim is to reduce hospital admissions. The matrons will also be providing care and support to patients that will be discharged from hospital that are medical unwell but stable and allowing them to be discharged back to their own homes earlier reducing the number of days that are staying in hospital.
The Community Matrons will work Mon-Friday and will be part of the wider multi-disciplinary team, working alongside Senior Nurses, Assistant Practitioners, Consultants, OTs, Physios, Pharmacists, Secondary Care and Social Services.
Main duties of the job
As a community matron you will undertake a variety of tasks and responsibilities, including:
The Non Medical Prescribing V300 qualification is desirable, but willingness to undertake the course within 1 year of appointment to post is essential.
- Carrying out comprehensive physical examinations.
- Planning and carrying out treatment, including the prescribing medicines, or refer patients to an appropriate specialist.
- Using their expert knowledge and clinical judgment to identify the potential diagnosis and where appropriate make a final diagnosis.
- Using their extensive practice experience to plan and provide skilled and competent care that meets patients' health and social care needs, involving other members of the healthcare team as appropriate.
- Ensuring the provision of continuity of care, including follow-up visits.
- Assessing and evaluating, with patients, the effectiveness of the treatment and care provided and make changes as needed.
- Providing leadership, supervision and mentorship to colleagues in the team and students.
- Making sure that each patient's treatment and care is based on best practice.
- Working as part of multi-professional and multi-agency teams.
This role is working 22.5hrs (3 days) between Monday and Friday 9am-5pm

Job Purpose
Virtual wards support patients, who would otherwise be in hospital, to receive the acute care, remote monitoring and treatment they need in their own home or usual place of residence, otherwise known as Hospital at Home. Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. The overriding purpose of the Community Matron is to support the provision of the highest quality patient care to individuals whilst on the virtual ward pathway.
The post holder will support the / virtual ward registered staff and clinical team lead ensuring the delivery of high-quality patient care upholding patient dignity delivered with kindness and compassion.
Provide and monitor the nursing care for individuals on the virtual ward pathway, providing expert clinical case management for patients on the virtual ward with multiple long-term conditions, who are at risk of readmission. Plan, co-ordinate and deliver care as appropriate per the daily MDT, initiate and manage medical and nursing care interventions, lead care and co-ordinate partner agencies and stakeholders
Have a clinical role, with responsibility for planning, managing and co-ordinating the care of people with highly complex needs and long-term conditions within a defined caseload.
Role Context:
- To provide appropriate and effective care management through the virtual ward pathway unnecessary hospital admission from the Community managing the active caseload and providing necessary oversight and support
- To comprehensively holistically assess the needs of patients and carers, plan, implement and evaluate nursing care interventions
- Embrace multi professional and partnership working to facilitate co-ordination, promotion of optimum health and social outcomes independence and self-care
- Undertake virtual monitoring and escalate concerns to the daily MDT
- Will be responsible for holistic assessments, implementation of care, overall co-ordination of self-care management or care management
- Oversee junior members of the virtual ward team
Key Accountabilities
CLINICAL PRACTICE
- Using expert knowledge, advanced nursing and autonomous decision-making skills, manage a caseload of patients in the community/ Residential Care home setting
- Comprehensively and holistically assess, review and evaluate the needs of both patients and their carers to improve their physical and psychological wellbeing whilst reducing acute exacerbation of underlying conditions and need for hospitalisation
- Undertake virtual monitoring and use technology to help determine the health status of individuals
- Undertake clinical duties and maintain own clinical practice and competencies to a high standard
- Ensure that patients are assessed, treated and monitored in the most appropriate care setting for their needs at that time
- Initiate and implement pro-active care plans

- Uses systematic processes of reasoning and reaches justifiable conclusions, including the rationale for establishing a differential diagnosis.
- Clinical monitoring of long-term conditions and appropriate treatment / actions in response to clinically significant changes in condition.
- To develop advanced specialist clinical skills to undertake interventions consistent with evidence-based practice in line with national and local policy

- Maintain contact with the patient if she/he is admitted to hospital and give the unit treating the patient the information they need to ensure integrated and consistent care is maintained.
