Job summary

An exciting opportunity has arisen for a Head of Clinical Services in Nottingham CityCare Partnership providing leadership, operational management and service development to the Integrated Reablement at Home service, Clinical Beds team, Front Door Integrated Discharge Team and support to the Transfer of Care Hub to meet the needs of our diverse population.
We are looking for an experienced leader with vision and passion, who can inspire innovative ways of working with key partners to support the development and delivery of Discharge to Assess and hospital avoidance services to meet the needs of our diverse population

Main duties of the job
Strategic leadership and operational management responsibility for Integrated care
Working effectively ensuring performance management and quality systems are in place and maintained across services
Lead the NCCP workforce in realising and demonstrating core vision, values, and improved outcomes for citizens
Lead Clinical services and teams
Lead transformation across services
