Job summary
The post holder will work as part of the Primary Care Networks (PCNs)
Multi-Disciplinary Team across member practices with specific responsibilities
to liaise with and support Primary Care Colleagues to ensure seamless care and
assessment. Provide advanced triage process directly to GPs, directly liaise
with GPs and act as a single point of contact for the PCN where there is
uncertainty about the level of mental health need that requires further
exploration.
Act as a
liaison point between primary and secondary care services, including liaison
with Care Coordinator where the service users presenting in primary care is
already known to Navigo's community mental health team.
Work
as an active member of the PCN MDT undertaking supervisor role
Main duties of the job
Communicate complex clinical and non-clinical
information in a non-judgemental and understandable way to a variety of
stakeholders, service users and other agencies.
Promote and
foster a culture of active engagement and co-production across the PCN
community & system wide.
Liaise with
other health and social care staff from a range of agencies in the care
provided clients.
Attend
multi-disciplinary meetings, Professionals Meetings, Complex Case Meetings and
others relating to referrals or Service Users in treatment, where appropriate.
Use highly
developed communication skills in working with people to understand their
personal and often very sensitive difficulties and develop carefully tailored
interventions.
Use highly
developed communication skills in working with external agencies/referrers to
understand their concerns relating to mental health and ensure that the right
interventions are provided across North East Lincolnshire
