An exciting opportunity has arisen for a Support Worker position within our Older People's acute services to support our service users with all aspects of their care including activities and personal care.

Please note; the expected start date for this position is not before 05/1/2026.

Main duties of the job

This role focuses on the direct needs of service users by delivering individual treatment plans across boundaries of care.

Job description

Job responsibilities

The Post Holder must have the individual service users needs at the fore at all times and use the agreed set of values and skills to underpin their day to day work.

Supervise and support junior staff (Band 2) in their roles, providing advice and acting as a role model and mentor.

To work without direct supervision, but with someone available to refer to, as and when required, either on or off site on a regular basis, and to take charge of an area in the absence of qualified staff, making judgements around service users conditions, at that time, and the requirements of the area.

Ensure that service users engage effectively with the agreed care plan and across appropriate services provided on a regular and consistent basis.

Will plan and supervise individual or group therapy sessions, in line with the agreed care plan or programme of care.

Provide regular and practical support to service users and their carers/significant other in developing and managing dignity and independence.

Provide support and assistance, as necessary, with daily living activities including personal care.

The Post Holder will be involved with facilitating community and hospital appointments when necessary.

Positively promote independent living of service users within the community.

Work flexibly with Key Worker, Care Co-ordinator, other relevant community support, service user and significant other to implement plan of care.

Will be handling service users cash and valuables on a regular basis.

Help identify early signs of relapse by monitoring the services user progress, level of functioning and mental state and alert the appropriate staff involved in the persons care.

Assist the Care Co-ordinator/Key Worker and service user in developing crisis/contingency plans and advanced statements, as per local policy guidelines.

To maintain adequate records, as required, by existing procedures, entering appropriate details on the service users case notes, as necessary.

To be able to communicate routine information in an arena where there will be barriers to understanding, and to be able to use tact and persuasion in a non aggressive manner.

Provide information on health promotion and wellbeing.

To ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individual.

Attend and participate in agreed training, development and supervision sessions as an individual and as part of the team.

To undertake such other duties as may be determined from time to time within the general scope of the post.

Qualifications

  • NVQ Level III or Certificate in Community Mental Health, or a willingness to undertake this, or an equivalent level of experience.

Desirable

  • A basic knowledge/understanding of the Mental Health Act and Mental Capacity Act
