Senior Eating Disorder Practitioner - Community

Job summary

This role is part of a new project called Alternatives to Admission which will form part of the Community Eating Disorder Service. The aim is to prevent people from having to come into hospital and support them within their own home instead. This will involve working with the persons family and loved ones and other agencies in order to provide a holistic approach to a persons care. This project will also enable people who are in hospital to be discharged sooner with support.

Main duties of the job

The successful candidate will take an active lead in developing and embedding; quality, evidence based, professional care within the Alternatives to Admission pathway. Close links with the Community Eating Disorder Service and YMM Alternatives to Admission colleagues are essential to ensure patients receive the care and treatment they require to promote safe physical and mental health. The post holder will plan and manage complete episodes of care as named keyworker; undertake independent comprehensive assessment and management of service users making complex clinical decisions regarding service user management and clinical outcomes. The post holder will be required to take a lead role in the MDT when planning alternative to admission care plans of comprehensive home treatment and meal support. The postholder will support safe and effective home treatment or the admission or discharge of patients as and when required .

Job responsibilities

Provide a quality specialist service to people (and their families/carers) referred to the alternatives to admission eating disorder pathway.

Promote the reputation of the service and support in the development of the Alternative to Admission pathway within Adult Community Eating Disorder Services.

Provide specialist assessment and intervention for people presenting with an eating disorder.

Provide timely response, according to urgency of need.

Manage a defined caseload, using evidence based/patient centred practice to assess, plan, implement and evaluate interventions.

Act as a keyworker for complex cases needing the support of the alternatives to admission pathway.

Deliver care within an extended hours provision as and when required to support adults and their families and improve access to services.

Develop and provide training for stakeholders and other organisations.
