Team at Navigo in Grimsby.
No hospital
experience required! We provide access to training that would form a pathway to
become a specialist clinical pharmacist.
Our clinical
pharmacy service is growing and we are looking for a pharmacist to support safe
use of medications within our organisation.
The post
holder will provide leadership on medicines management and optimisation, and
quality improvement and manage some aspects of the quality and outcomes
framework.
Please note
the attached Job Description is based on band 7 role, however, should band 6 be
agreed, it will be reviewed to reflect the scope of practice.

Main duties of the job
To provide
comprehensive medicines information and advisory service to other healthcare
professionals in order to ensure the best individualised choice of medication
for patients and avoid delays in treatment.
To ensure
parity of esteem is prioritised and that the physical health of patients is
maintained and monitored. To provide care plans to help improve physical health
further and liaise with organisations who can support physical health
improvements.
Work as part
of the multi-disciplinary team (MDT), liaising with other pharmacy team
members, nurses, doctors and other healthcare professions in order to resolve
patient centred pharmaceutical problems.
To
reduce the likelihood of service user (re)admissions resulting from medication
related issues. To provide support to the teams around safe discharge to
Primary Care and care planning. The purpose of this role will be to provide
comprehensive clinical pharmacy service to inpatient and community teams.
