Main duties of the job

The Health Care Assistant role at Navigo involves providing person-centred support to individuals with enduring mental health conditions within both inpatient and community rehabilitation settings. You will build therapeutic relationships, support daily living skills, and deliver 1:1 and group interventions to promote recovery, independence, and wellbeing. Key responsibilities include observing and reporting changes in mental state, facilitating practical and emotional support, and contributing to care planning. The role requires emotional and physical resilience, flexibility, and a compassionate, proactive approach. About the Role

This is a privileged and highly rewarding position centred on building meaningful, therapeutic relationships with our service users individuals who may be stepping down from secure placements or who require tailored support in the community. You will be part of a multidisciplinary team working within both inpatient and community rehabilitation pathways.

Your role will involve:

Working closely with a small caseload of service users to provide holistic, day-to-day support.

Advocating for service users to ensure their voice is heard and that their care plans reflect their unique needs and aspirations.

Supporting recovery through structured 1:1 and group interventions.

Empowering individuals to develop essential life skills, including cooking, budgeting, managing health and well-being, and participating in meaningful activities.

Observing and recording changes in mental state and behaviour and reporting these to senior staff for care review and adjustment.

Promoting hope, optimism, and self-efficacy in all aspects of recovery.

Key Responsibilities

Person-Centred Support:

Develop meaningful, supportive relationships and provide tailored care to help service users gain confidence, autonomy and the skills needed for daily living.

Delivery of Interventions:

Facilitate both 1:1 and group activities focused on practical and emotional wellbeing, including:

Budgeting, welfare, and benefits support

Healthy living and nutrition

Vocational and educational planning

Coping strategies and stress management

Engagement in community and leisure activities

Observations and Reporting:

Monitor and report any changes in mood, interaction, mental state, and daily functioning, helping to prevent relapse and contributing to the overall care planning process.

Recovery-Focused Work:

Encourage goal setting and support service users in achieving their potential through structured support, compassion, and guidance.

Record-Keeping:

Accurately document interactions, observations, and interventions in our electronic patient record system.

Team Contribution:

Participate actively in team meetings, training sessions and service development discussions. Share insights and contribute to a positive and collaborative team culture.

Work Flexibility:

Youll work across inpatient settings and community environments such as service user homes, kitchens, gardens, gyms or outdoor activity spaces wherever care and engagement is needed.
