Job summary
The
Community Urgent Care Team (CUCT) provides a comprehensive 24/7 health and
social care triage service to patients and/or their carers/relatives with the
aim of averting the need for hospital admission or early hospital discharge.
This
includes assessing complex and specialist needs in order to develop, implement
and evaluate the appropriate programmes of care and wherever possible,
supporting individuals to remain in a community setting whether it be their own
home, a residential home or nursing home.
The CUCT will continue to be involved with the individuals programme of
care for up to 7 days if required.

Advanced Urgent Care Practitioner
Community Urgent Care Team
Westgate Park, Grimsby
Permanent - 37.5 hours per week
You will be required to work a rota pattern of
12 hour shifts including weekdays, nights, weekends and bank holidays.
Salary Range 7 £45,987- £52,434 per annum
You will be
practising as an Advanced Clinical Practitioner (ACP) providing a first point
of contact/triage for patients with undifferentiated and undiagnosed health
problems. You will work
predominantly in an autonomous and independent manner and will assess, exam,
diagnosis and implement a plan of care/treatment which may include follow up,
discharge or referral to other health care professionals.
This exciting and challenging role offers you the opportunity to provide high quality patient focused care in the community,
providing clinical care. You will be responsible for health assessments
including exacerbations, undifferentiated and undiagnosed of acute and chronic
conditions. You will develop an understanding and application of social
assessments in crisis. Community
experience is desirable, however appropriate training and development will be
offered to those with a wide range of transferable skills and experience.
