Job summary

The Community Nursing Service within North East Lincolnshire provide nursing care and support to patients, families and carers in a variety of community settings supporting patients to remain in their own homes, preventing hospital admission and facilitation of early hospital discharges and managing end of life care.

Our Community Nursing staff work closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurse and other healthcare staff to provide a quality service personalised to meet individual needs. The service supports and assists patients to manage ill health and long term conditions including elements of disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect.

Main duties of the job

Job title: Community Staff Nurse

37.5 hours Fixed Term (9 months maternity cover)

Based: The Pilgrim Primary Care Centre, Immingham

You will be required to work a rota pattern which includes weekends and Bank Holidays

Salary Range 5 £30,805 - £35,715 per annum

The post holder will be appointed on a fixed salary point within the salary range

The post holder will be responsible for the delivery of skilled nursing interventions for patients on the community nursing case load maximising their individual health by providing both physical and emotional support.

The postholder will identify patient needs and refer to appropriate health and social care agencies and other professionals. They will support, and in the absence of more senior community nursing staff be responsible for the assessment of clinical needs and the development, implementation and evaluation of individual plans of care.

The post requires you to transport yourself and equipment across locations within North East Lincolnshire and to deadlines. Therefore a flexible approach to your working arrangements is essential.
