Job summary
The Community Inpatient Unit provides
residential rehabilitation focusing on a multi-disciplinary approach providing
a holistic assessment of needs. A bed-based Enhanced Recovery service which provides Rehabilitation and
re-ablement opportunities for adults in North East Lincolnshire. The service
will promote faster recovery from illness, prevent unnecessary acute hospital
admissions and premature admissions to long-term care, support timely discharge
from hospital and maximise independent living wherever possible. Multi-professional
staff will collaborate with individuals to ensure they achieve their full
potential in terms of their level of independence, health and wellbeing.

Base: Community Inpatient Unit
Hours: 30 hours per week, permanent, Monday - Friday
Salary Range 6 £36,806-£44,717 pro rata per annum.
We are looking for a
senior social worker to join our team who is committed to providing a
strengths based approach and passionate about supporting patients to achieve/
regain independence.
As a senior social worker
within The Community Inpatient Unit you would be expected to undertake needs
assessments in line with The Care Act (2014), complete Mental Capacity/ Best
Interest assessments, design and arrange support packages to facilitate safe
and timely discharges and work closely with our nursing and therapy teams
amongst other social work functions. .
