This is an exciting opportunity to join a well-established specialist community service the Intensive Support Team (IST) in NE Lincolnshire to provide Forensic Case Management for service users within the Forensic Outreach Liaison Service (FOLS). The FOLS Team are a specialist community based service that supports people with Learning Disabilities and/or Autism who are also offenders. The purpose of the FOLS Team is to enable these people to move back into their local area from out of area Forensic Services, as well as support local offenders with LD and/or Autism to live safely in their local communities instead of having to go out of area into secure services.

Following the shortlisting process, successful applicants will be invited to deliver a formal presentation, which will be followed by a panel interview at the Val Waterhouse Centre, 4143 Kent Street, Grimsby, DN32 7DH.

Band 7 £45,987 - £52,424 per annum, depending on skills and experience

The local NE Lincs FOLS Team is part of a wider FOLS Service working beside HUMBER, which means you will also be working alongside Specialist Forensic Services in Hull to support Transforming Care partners to repatriate offenders with LD and/or Autism and complex needs back to their local communities.

You will be expected to manage the transition process from out of area to local provision, using a multi-disciplinary (MDT) model that will include attending and/or chairing MDT meetings both within and out of area, liaising with various professionals and organisations to establish and enable effective partnership working.

Relevant professional qualification and registration with working knowledge of the Mental Health Act and health and social care pathways is essential as well as experience working with complex cases in the field of Learning Disability and/or Autism.

Communication and organisation skills are also essential as well as the ability to work autonomously using your own initiative and as part of a Multi-Disciplinary Team working in partnership with other services, agencies and organisations to meet the Transforming Care targets and to provide a successful community based forensic support service that improves the health and wellbeing of local people.
