Job summary
The Urgent Care Team is a 24/7 service providing a first point of
contact and comprehensive triage service, telephone and clinical assessment to
patients and/or their carers/relatives resulting in a safe and appropriate
clinical decision and plan of care.
The Urgent Care Team is responsible
for health and social care triage including supporting aversion of hospital
admission or early hospital discharge through assessment and treatment of
individuals in crisis, assessing complex and specialist needs in order to
develop, implement and evaluate programmes of care. Supporting individuals to remain in community
settings, in their own home, residential home or nursing home for a short
period of time.

The
Urgent Care Assistant provides a short term crisis intervention service to
individuals within their own home environment that could include personal care
and any support that has been identified through the assessment process.
You will work in partnership with nurses, physiotherapists, occupational therapists or care managers across the Intermediate Tier Service and undertake delegated duties after appropriate instruction in conjunction with a detailed support/outcome plan.
You will need to be able to work well as
a team as well as using your own initiative and you will need to have good
communication and customer care skills.
It is essential that you have 2
years experience of working in a health and social care setting and have
knowledge of the principles of enablement and self-care principles.
