Job summary
The
Specialist Palliative Care Team is a health and social care team that provides
an expert clinical and social care service for patients with challenging and
complex life limiting diagnoses and their families. All senior team members act
as clinical or social care experts, leaders, and educators for their service
areas. The team provides a service in all care settings across the North East
Lincolnshire locality.

Main duties of the job
Job Title: Assistant Social Welfare Officer
Department: Palliative
and end of life care
Base: St. Andrews Hospice, Peaks Lane, Grimsby
Hours: 18.75 hours per week, permanent.
You
will be required to work between hours of
9-5 pm weekdays, weekend. Bank
holiday working is not required.
Range 4 (£26,416 - £28,182) pro rata per annum - you will be appointed on a fixed point
within the salary range.
The social welfare team is a small friendly
busy team working with patients and families to maximise their income,
alleviate poverty and enable best quality of life. The team provide support to
access benefits, apply for grants and advice with issues of housing, funeral
poverty, and benefit appeals.
If you have a relevant degree, recent
experience in a social welfare role or
advisory post, are a team worker,
enthusiastic about supporting individuals to maximise their income and have a
good knowledge of the benefits system,
funeral poverty, excellent IT skills and the ability to complete complex
calculations this post could be for you?
You will need experience and the ability to
network with other agencies voluntary, statutory and charities. You will also contribute to the development
of national agendas through the work of Macmillan Cancer support and Integrated
care systems locally.
