Fraility Information Care Coordinator

Job summary

The co-ordinator will work predominantly in the Urgent and Emergency Care arenas within the Hospital, alongside the multi-disciplinary teams within those environments. The Co-ordinator will be part of the Hospital Discharge Team which support discharge and patient flow, focusing on the needs of the frail person.

Frailty Information Care Co-ordinator

Department: Hospital Discharge Team

Based at: Diana Princess of Wales hospital, Grimsby.

Hours: 37.5 hrs Fixed term for 12 months

Predominantly Monday to Friday but able to flexible to meet the demands of the role

Range 4 £25,498 - £27,202 Per Annum. You will appointed on a fixed salary point within the range.

This is a new and exciting role based at Diana Princess of Wales hospital.

The co-ordinator will work between multiple organisations involved in the health, social and holistic care of patients who are recognised as frail, extracting and coordinating information for frail patients on attendance, to help inform clinical decision making and care planning for frail individuals, including redirection back to the community.

The information gained from the patient, family, carers, electronic patient information records will help to identify and inform the right care by the right person at the right time to ensure best outcomes for the individual.

The Co-ordinator will be a strong team member, demonstrating ability to work with a wide variety of people at different levels of seniority and from different professional disciplines as well as working independently. Attention to detail, digital literacy and communication skills are vital in this role.

  • Foundation Degree or Higher Education Certificate (level 4) in Integrated Care or other appropriate topic
  • OR
  • Level 3 in health or social care, plus 2 years experience in this field and with willingness to work towards level 4 on appointment
  • Literacy and numeracy level 2 qualification
