Job summary
The Hope Specialist Falls and Respiratory Service provides a
patient-centred one stop shop for people with chronic respiratory diseases,
such as Chronic Bronchitis and Emphysema (COPD), and older people at risk of
falls. From its inception, the service has been designed and developed through
a co-production approach between a multi-disciplinary specialist team and its
users and offers individually tailored and holistic assessment, treatment, rehabilitation
and education within a community setting.

£24,100 per annum. You will be appointed on a fixed salary point within the salary range.
You will provide assistance to the therapy-led Hope Specialist Service
multi-disciplinary team (MDT) in organising and delivering efficient, effective
patient care. This will include carrying out delegated clinical work and
administrative and clerical duties within the service. Some clinical work will
be unsupervised.
In addition, you will raise
awareness of the benefits of good health and give practical support to help
people improve their knowledge, skills and confidence in improving their
lifestyles and managing their conditions by using community development
principles and values to address and reduce health inequalities in North East
Lincolnshire.
You will be educated to Literacy, Numeracy and IT level 2
standard and have an NVQ Level 2 in the appropriate field e.g. Health and
Social Care (or willing to work towards this).

- Previous experience in a health care or equivalent setting which may include therapeutic support work
- Understanding of local community issues in North East Lincolnshire
- Experience of collaborative and multi-agency working
- Experience of working in a community setting
Desirable
- Experience and competency of working as an assistant / technical instructor or their equivalent.
- Understanding of medical records and therapy records keeping
