Job summary
The NEL Community Therapy Team provides Occupational Therapy and Physiotherapy to support people with health and social care needs to stay in their own home and help them remain independent. Community Therapy staff are employed by Care Plus Group and Northern Lincolnshire and Goole NHS Foundation Trust working in partnership and provide direct clinical interventions through assessment of need and person centered goal planning. The range of interventions includes treatment, rehabilitation, minor and major housing adaptations, loan of equipment and moving and handling solutions for children and adults with a primary physical disability.

The successful candidate will assist clinical staff in providing Occupational Therapy assessment and intervention to physically disabled residents living in North East Lincolnshire thereby helping to maintain people's independence and improved health and wellbeing. You will provide a service to deliver, install and collect equipment within the local area.
You will carry an allocated straightforward caseload, alerting senior staff where urgent action may be required due to information received. You will work in patients own homes, often as lone practitioner, to follow treatment plans as directed by registered staff.
You will provide relevant intervention within own range of competency using a combination of techniques, rehabilitation and equipment and adaptations to maintain, restore and promote independence in the community. You will identify areas of need beyond own competency level and seek guidance from OT in order to provide people with information about relevant resources.
To be considered for an interview you must have the following:
NVQ Level 2 In Care or similar
Level 2 numeracy and literacy or equivalent level of competence
Level 2 IT skills or equivalent level of competence
Previous experience in an enabling/ rehab field as a support worker
