Job summary
The
Community Nursing Service within North East Lincolnshire provide nursing care
and support to patients, families and carers in a variety of community settings
supporting patients to remain in their own homes, preventing hospital admission
and facilitation of early hospital discharges and managing end of life care.
The Community Nursing Service works closely and in partnership with Primary
Care Networks, Secondary Care, Social Care, Specialist Nurses and other
healthcare staff to provide a quality service personalised to meet individual
needs. The service supports and assists
patients to manage ill health and long term conditions including elements of
disability, thus allowing them to have the best possible quality of life, while
maintaining dignity and respect.

We are looking for an experienced Registered General Nurse with an additional post registration qualification and experience in palliative care, wound management, leg ulcer management, continence care and diabetes care. You will be responsible for the day to day running of the team, therefore some experience of supervising staff and dealing with staffing issues is essential.
Most importantly you must be able to demonstrate your commitment to the values of Care Plus Group.
The post requires you to transport yourself and equipment across locations within North East Lincolnshire and to deadlines and you will be required to work evenings, weekends and bank holidays on a rota basis. Therefore a flexible approach to your working arrangements is essential.
