Job summary
The Urgent Care Team is a 24/7 service; providing a first
point of contact and triage service, including telephone and face to face
clinical assessment of patients and/or their carers/relatives, resulting in
appropriate decision making and plan of care. The Team work closely with other
disciplines including GPs, District Nurses, Mental Health Services, Adult
Social Care, End of Life Care Services, Paramedics and other external
providers.
The Triage Clinicians, as part of the Urgent Care Team, are
a dynamic group of Nurses and Paramedics with a diverse range of experience and
backgrounds, which responds to the unplanned needs of the patients in North
East Lincolnshire.

Salary: Range 6 £33,675 - £40,439 pro rata, per annum (New Salary Range 6 £35,527 - £42,663 Including 5.5% Pay Award).
To
provide a high standard of clinical assessment both on the telephone and face
to face to patients within North East Lincolnshire, predominantly but not
exclusively, in the Out of Hours period (evening, weekends and bank holidays).
As a senior clinician you will act as
shift lead and assume supervision of team members whilst on duty, manage rota
and staffing levels, share clinical knowledge and help with the delivery of
training and education. You will work closely with Adult Social Care and Mental
Health Teams within the Single Point of Access in an MDT role.
By utilising agreed protocols and policies in
conjunction with listening, questioning and probing skills you will support in
planning individual patient care. You will complete clinical assessment of
minor ailments through to life threatening emergencies and arrange appropriate
and safe Ambulance and GP attendance, if required.
