The Community Nursing Service in North East Lincolnshire works in liaison with GPs Practice nurses and other agencies to provide a high quality, skilled and holistic nursing care service to the community.
We
are looking to appoint a qualified Registered Adult Nurse 1st Level with
additional post registration nursing qualifications at Advanced Practice MSc
level including V300 Independent prescribing and Long Term Conditions, along
with community experience.
You
will be expected to work closely with the Community Nursing Service, Primary
Care Networks, Secondary Care, Social Care, Specialist Nurses and other
healthcare staff to provide a quality service personalised to meet individual
needs.
As Community Advanced Practitioner (CAP) you will
be part of a highly experienced team of nurses who work closely with patients
with a chronic long term unstable conditions.
The aim of the Community Advanced Practitioner Service is to:
-Provide
case management to patients of a complex nature
-Help
managing episodes of deterioration in health
-Ensure
Patients feel supported at home in the management of their health
-Increase
the range and responsiveness of community services
-Plan
for, predict, and prevent crises in care management.
Main duties of the job

Hours: 32 hours per week
- you will be required to work a rota pattern including weekends/Bank
Holidays.
Contract: Permanent
Salary Range 7 £41,617
- £47,499 pro rata per annum

As part of the community advanced practitioner team
you will have a variety of tasks and responsibilities dependent on patient
need, these include:
Carrying out physical
examinations
Deciding on and
carrying out treatment, including prescribing of medicines, or referring
patients to an appropriate specialist
Using expert
knowledge and clinical judgement to identify potential diagnosis and make a final diagnosis
Using extensive
practice experience to plan and provide skilled, competent care that meets
patients health and social care needs, involving the
healthcare team as appropriate
Ensuring the provision
of continuity of care, including follow-up visits
Assessing and
evaluating, with patients, the effectiveness of the treatment and care provided
and make changes as needed.
Liaise with health professionals to offer additional help and expertise with patient
consent.
