Job summary

The Haven team provides Health & Social Care services to palliative/end of life care patients and families in their own homes. Day services are available between 8am -10pm and night care 10pm 8am.

The service provides 24/7 health and social care support for people who are at the End of their Life and need support to remain at home as well as those who have an advanced and progressive palliative condition that require short term support due to an unstable event or crisis situation.

The service is a support service working in conjunction with the community nursing teams, however the service will provide registered nursing support and some of the patients referred to the service will be case managed by a registered nurse.

The team will provide a mix of health and social care to the patient and their family to enable people to meet their preferred priorities of care and preferred place of death.

The service will work through a central point of referral and care support will be agreed by the support at home co-ordinator.

Based at St Andrew's Hospice

37.5 Hours.You will be required to work a rota pattern which includes weekends and Bank Holidays, Shifts are a combination of 8am-4pm & 12-8pm, and 8am- 8pm and includes weekends

Salary Range 6 £33,675 - £40,439 pro rata per annum

The post holder will be responsible for the day to day delivery of nursing & social care needs, support and respite care to patients and their families in the community.

You will work collaboratively with community nursing teams, Hospital Discharge Team and Hospice to assess patients and families with Palliative and End of Life Care needs for eligibility to access services provided by the support at home team.

You will accept referrals to agreed service criteria working closely with the team co-ordinator to plan, implement and review service provision.You will also be responsible for the day to day supervision and support of the senior care workers and care support workers within the support at home team.

You will provide psychological and emotional support to the team carers who are often dealing with highly complex, emotional and distressing circumstances with patient and carers in palliative /End of life care and bereavement situations.

You will provide a high level of clinical knowledge including pain & symptom management and will be able to administer appropriate medications as prescribed if required.
