Care Plus Group Primary Care practices Open Door and Quayside provides the first point of contact for registered patients with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect in the provision of health and social care.

Main duties of the job
Job Title: PCN Care Co-ordinator
20 hours per week

£23,603 pro rata, per annum - fixed salary point
You will work closely and in partnership with the Primary Care Network clinical team, coordinating resources and information to support choice, access and delivery of personalised care.
Care Coordinators will be responsible for consulting with patients and determining their needs, developing care plans, coordinating patient care services, education regarding their conditions and working with the medical team to evaluate interventions.
You will be responsible for consulting with patients and family members to discuss a variety of issues such as health and social problems. Supporting patients in their understanding and management of their conditions and medications.
You must have the ability to work with minimum supervision and possess effective computer skills.
