Job summary

Care Plus Group Primary Care practices provides the first point of contact for registered patients accessing the health care service. The service will work collaboratively with patients to assist them in managing and preventing illnesses and coordinating access to specialised care.

The Care Plus Group Primary Care practices, Open Door and Quayside, provide services for a client group with a range of diverse and sometimes complex needs including substance misuse, homelessness, mental health problems and self-neglect.

Main duties of the job

Job Title: Bank Advanced Nurse Practitioner

Hours: Bank hours to cover maternity leave

Monday Friday

Salary dependent on experience you will be appointed on a fixed salary point within the salary range.

You will act as an autonomous practitioner working independently and in conjunction with other healthcare professionals within a range of primary care facilities to assess, diagnose and treat the conditions of those patients attending the service.

You will provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence and will undertake Clinical Nursing Practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing services.

You will assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis and complex needs. Clinically examine and assess patient needs from a physiological and psychological perspective, plan clinical care accordingly and deliver care according to NICE guidelines. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

You will provide professional and quality administrative support and reception service to patients, doctors, other health professionals and all visitors and work under general supervision and within well established procedures.
